    Please ensure this is given to the referee at least five minutes before kickoff. Sections indicated * must be completed. 
    Amateur Football Combination rules allow for three substitutes to be nominated, and under the Repeat Substitutes rule, 
    a player who has been substituted becomes a substitute, subject to having been named to the referee prior to kickoff. 
    Any player not named to the referee as a substitute prior to kickoff is not eligible to be used as a substitute.
