JUDICIAL AFFIDAVIT OF MEL QUIAMBAO

I, MEL QUIAMBAO, of legal age, single, and living at 123 Hermanos St.,
Caridad, Quezon City, petitioner in this case, state under oath as follows:

PRELIMINARY STATEMENT

The person examining me is Atty. Jessica S. Vicentino with address at 117


Gamboa St., Sta. Mesa, Manila City. The examination is being held at the
same address. I am answering his questions fully conscious that I do so under
oath and may face criminal liability for false testimony and perjury.

PURPOSE

This affidavit of petitioner MEL QUIAMBAO is being offered to prove that


the respondent has refused and continuously refused to give financial support
to their child.

Q: State your name and other personal circumstances for the record.
A: I am Mel Quiambao, 28 years old, single, and with residence at 123
Hermanos St., Caridad, Quezon City.

Q: Do you know the defendant in this case?


A: Yes.

Q: How did you know the defendant?


A: He is my former boyfriend.
Q: How long have you been together?
A: Two (2) years.

Q: How many kids do you have with the defendant, if any?


A: One.

Q: Describe your relationship with your former boyfriend.


A: The first two years of our relationship was harmonious. Until he decided
to work abroad sometime in May 2018. We decided to part ways on July the
same year because of our long distance relationship set-up.

Q: What happened after you parted ways?


A: After we parted ways, I didn’t know I was already pregnant with our child.

Q: How will you prove that he is the father of your child?


A: I was already three (3) months pregnant when we parted ways. We have
an exclusive relationship for two years. Both our families know our
relationship. In fact, his parents acknowledged my daughter as their
grandchild.

Q: Aside from that, do you have any proof to support your claim?
A: Yes. I already filed a petition for filiation which was granted August 2019.

Q: After you learned that you were pregnant, what did you do?
A: I didn’t do anything after I learned my pregnancy. However, after giving
birth I decided to reached out to him, to tell about our baby and to ask for
financial support. Unfortunately, I could no longer contact nor locate his
whereabout. It was just June 2019 when I heard a news that he was already in
the Philippines.

Q: What did you do when you learned that he was already in the Philippines?
A: I contacted him to set-up a meeting with him together with our baby. So
that I could finally introduced her to Greg and to discuss on the probable
scheme for child financial support.

Q: What happened after that?


A: He didn’t show any interest in meeting up and he even refused to give
financial support and denied our child, saying that he is not the father.

Q: Did you tried reaching out to him again?


A: No.

Q: What did you do instead?


A: I filed a case to obliged him to give financial support to our child.
Q: Is there anything you want to add to your testimony?
A: No.

ATTESTATION CLAUSE

I, ATTY. JESSICA S. VICENTINO, of legal age, Filipino, with postal


address 117 Gamboa St., Sta. Mesa, Manila City, after being duly sworn
depose and say:

1. I was the one who conducted the examination of Mel Quaimbao at my


aforementioned office in 117 Gamboa St., Sta. Mesa, Manila City;

2. I have faithfully recorded or caused to be recorded the questions I asked


and the corresponding answer that the witness gave;

3. I nor any other person then present or assisting her coached the witness
regarding her answers;
