WITHOUT PREJUDICE

RE: Outstanding Balance for Payment of Loan Account 35736


We write in behalf of our client, GEJ Financial Services, Inc., the
matter of your non-payment of your obligation.
This letter will serve as formal notice that you are in default of your
obligation to pay the sum of Twelve thousand pesos (PhP 12,000.00) which
was loaned to you on January 31,2019 plus interest in the amount of 12% per
annum. The total amount of Thirteen Thousand Four Hundred Forty Pesos
(PhP 13,440.00) is now overdue and you have failed to repay the same despite
repeated requests for payment.
Your outstanding account have been overdue for an extended period of
time and have therefore been handed to the undersigned for further action.
We feel we have been extremely fair in our efforts to collect the above
noted account. Despite these efforts, you have failed to bring the account(s)
current. Accordingly, we hereby demand that a payment, in the amount
noted below, be paid no later than 15 DAYS. Failure to comply with this
request will cause us to take further action to collect this outstanding
amount which could include litigation.
If we do not receive payment, this account(s) will be forwarded to
credit reporting and collection agencies, which will negatively affect your
credit rating and may result in court proceedings.
We trust that you will give this matter your prompt and preferential
attention to avoid the expense and inconvenience of litigation
If payment has already been made, please disregard this notice.

I, Dra. Belinda G. Eillish, Filipino, of legal age, and a resident of 726
Pablo Ocampo Sr. Ave, Malate, Manila, Philippines, having been duly sworn
in accordance with law, hereby depose and state:
1. That I am a duly-licensed medical doctor, practicing at Philippine
General Hospital with Professional License No. 324503 issued on
November 11, 2011 at Manila;
2. That on or about 12:00 midnight of February 14, 2014, I personally
attended to a patient named Monica Geller in giving birth by normal
delivery to a baby boy whom they named as Phoebe; and
3. That I execute this Affidavit to attest to the truth of the foregoing
facts and for any other legal purpose that this Affidavit may serve.
