> 1 patient was brought to Rural Health
Unit for suturing due to laceration on
the left lower thigh back area.
> Some of the MDR have corrections
specially on their birthday and spelling
of name.
Unavailability of medicine for primary
care and for Hypertensive patients.
Some were not vaccinated due to:
 Presence of fever
 Baby in on vacation.
1 pregnant newly tracked during the
activity

Wound Cleaning and dressing done.
Assisted during suturing.
Advised member to visit the Philhealth
office for updating of MDR.
Advised to buy prescribed medicine and
instructed to come back if symptoms
persist or worsen.
 Advised mother to continue
paracetamol every 4 hours until the
fever sudside.
 There were 2 postpartum in
Barangay IV, 1 from Lemu Sur and 2
from Divisoria visited and provided
health services
Assessment done before administration
of influenza vaccine to avoid
complications.
1 MMR,1 PCV were given
MHO DMO IV
