Directions: Read the situation carefully and make a sample family care plan based on your identified family
health problems.
Situation: A student nurse was instructed by her clinical instructor to go on a home visit to Mrs. B and
her family. She obtained the following data: the family of nine lives in a two-room house of light materials
situated in a congested urban community about 5 km from the health center. Water supply comes from a public
well ½ km away. Garbage disposal is dumping in an open pit situated at the back of the house. Toilet facility is
closed pit, foul smelling and with plenty of flies situated at the back of the house.
Family income is P240.00 a day and three children are studying. The youngest child Rina, 3 y/o weighs
10 kg, looks pale, lethargic and markedly underweight and undernourished. Mother verbalized “ Rina is really
small in built since she was a baby. She is not fond of eating.” Rina also has scabies which mother believed to
be due to bad weather condition but two of the siblings were brought to the center a month ago for scabies
management. The medicine intake of the two siblings was not completed because it was expensive. The eldest
girl takes care of the younger siblings because Mrs. B is busy as a vendor. She hardly sees the needs of the
children.
