AFFIDAVIT OF LOSS 

That I, Muammar John S. Maquiling, of legal age, Filipino Citizen, and a resident of Iligan City, 
after having been duly sworn to in accordance with law hereby depose and say: 

1. That I am a depositor in Landbank Carmen CDO Branch; 

2. That I am the Authorized Managing officer of the joint venture;

3. That the above­mentioned bank issued to me passbook bearing No.3141­0852­54; 

4.  That sometime on the 12th of December 2019 the above­mentioned passbook was lost 
inside the premises of my car and was nowhere to be found; 

5. That despite diligent search, the said Passbook cannot be found and now beyond recovery;

6. That the same was never used as collateral in any private loan and if found we will surrender
it to the issuing bank; 

7. That I am executing this affidavit to attest to the veracity of the foregoing statements and for 
the purpose of informing the issuing bank regarding its loss for replacement and for 
whatever legal intents and purposes this may serve.
