WITH RELEASE-WAIVER AND QUITCLAIM

Authorization

1. I ​am currently ​/ was previously employed with LizardBear Tasking, Inc.

(“TASKUS”) for the position of [__________________________];

employment verification i.e. employment, visa application, etc.) ​with

Pursuant to said application, I have authorized the aforementioned organization
to validate any and all information I have provided in relation to my previous
employment with TASKUS. Likewise, I have authorized said organization, to

request TASKUS to confirm accuracy of the following personal data which form
part of my prior employment records:

a. Full Name
b. Position/Title/Level
c. Employment Dates
d. Employment Status

3. The abovementioned disclosure is required by _________________ as part of its

application evaluation process which includes the use of the above-enumerated
information in an independent investigation of my background and credentials
and for other legitimate purposes of the said organization, of which, I am fully
aware.

4. I hereby authorize TASKUS to validate, where applicable, any and all information
I may have provided to _____________________ and to confirm to the latter, the
accuracy of the abovementioned personal information as part of my application
evaluation.

5. I further hereby hold TASKUS free and harmless, from any and all actual or
alleged obligations, claims, demands, causes of action, liability, loss, cost,
expense, damage and/or injury I may claim against _____________________ for
failure of the said organization to comply with the DPA, its IRR and any and all
related issuances of the NPC. Likewise, I hold TASKUS free and harmless from
any and all actual or alleged obligations, claims, demands, causes of action,
liability, loss, cost, expense, damage and/or injury I may claim against
_________________________ in relation to this authorization, the consequent
disclosure of my personal information resulting therefrom, and any unauthorized
processing and/or disclosure of personal information
________________________ may commit thereafter.

6. I hereby acknowledge that my personal data as above enumerated as held by

TASKUS are true and accurate as to the time it was collected and/or processed.
Should I find any inaccuracies or should there be a need to update any
information thereto, I shall immediately inform TASKUS and
________________________ for proper correction and/or update of my personal
data. As such, I hereby hold TASKUS free and harmless from any liability arising
from any inaccuracies and/or outdated information in my personal data.

7. Further, I hereby understand and acknowledge that I have the right to withdraw
this authorization or subsequently object to any further disclosures of my
personal information at any time. However, I hold TASKUS free and harmless
from any liability arising from disclosures made to ______________________
pursuant to this authorization and prior to any official act of withdrawal or
objection.

8. I am executing this Authorization with Release-waiver and Quitclaim, with my

written conformity below:

a. For the purposes explained above and for whatever legal purposes it may
serve; and

b. As affirmation that I have been fully informed of and completely

understand and acknowledge the terms and extent of TASKUS’s
