PARENTAL CONSENT

Philippines, after having been duly sworn in accordance with law, hereby
depose and say THAT:

1. I am legally married to ______________which marriage was

3. My husband and our son will be travelling together for a vacation

going to Hongkong;

4. I am giving my consent as a mother to allow my husband who is the
