Good day!

I am currently assigned at the, HSSG, HPAF and residing at, Villamor Air Base,
Pasay City. Affront the room I am currently occupying is an open septic tank which caused
the nuisance or disturbance to the nearby occupants due to the foul smell coming from it.
In order to eliminate the foul smell inside my room, I am forced to utilize a deodorizer
spray all the time. Due to frequent exposure to spray, I developed asthma and was
aggravated by my continuous stay in my quarters.

I am writing this letter to respectfully address the foregoing matter before your good
office and humbly request to transfer me to another quarters located at Luzon Street, Col
Jesus Villamor Air Base, Pasay City.

Thank you for considering my request and assistance in this matter. I look forward
to your immediate positive response.
