I am submitting this letter to formally request for a one-month Leave of Absence, effective February 12,
2020. This request is because of medical reason.

My doctor required me to have a full bed rest because of my current pregnancy condition. I have
Placenta Praevia, which makes my pregnancy high-risk. This condition causes my bleeding and may lead
to miscarriage.
