RE: PROCESSING OF THE RETIREMENT OF BUSINESS REGISTRATION TO ALL

CONCERNED REGULATORY BODIES OF SI EN TCM INSTITUTE

To the client:

This letter is to confirm our understanding of the terms and objective of our engagement as well as
the nature and limitations of the services I will provide.

Our Responsibility and Deliverables

Our deliverables will be the processing of the retirement of Si En TCM Institute (the “Company”) to
Securities and Exchange Commission (SEC), Local Government Unit (LGU) and Bureau of Internal
Revenue (BIR).

Limitations

We understand that the intended purpose of the retirement is to cease the business registrations of
the Company to all concerned regulatory bodies to avoid paying unnecessary penalties. Thus, any
changes in material respect, you should inform us.

Fee Policy

Our professional fees for the processing of the retirement of business registration is Eighteen
Thousand Pesos Only (PHP18,000.00). Also, the Practitioner won’t charge any Out of Pocket
Expenses (OPEs) such as transportation fare, photocopy fees, etc. Other extended services/works
that will be incurred to complete the deliverables shall likewise be billed in addition to the professional
fee indicated herein. We are further proposed to bill you as follows:
