1.3 Management Review

This Health and Safety Plan is compiled as guidance to employees, subcontractors and others with
genuine interest, as to the structure, organization and arrangements currently in place, for securing
the health, safety and welfare of all personnel employed on the project and to others who may be
affected by their acts or omissions.
The Health and Safety Plan is a live document and is intended to reflect what is happening in
practice. It is therefore understood, that constant review may require alterations and amendments to
be made, in the light of changing circumstances and never less than biannually. This review will be
done by the Project Management Team and any amendments approved by the General Manager
During the life of contract, work carried out shall be in compliance with all Statutory UAE and
Local Health and Safety Legislation applicable at the time. Specific additional rules and
requirements of the employer shall be strictly adhered.

2.0 PROJECT DESCRIPTION

2.1 Project Title Construction of Schlumberger Accommodation “MLC – MEA”


Abu Dhabi
2.2 Scope of Work

The project comprises of construction of main accommodation building [6 Blocks – total 492
individual rooms with attached bathroom], central amenities building, Indoor sports facility, Staff
Accommodation and external pavement. Total built –up area = 29,500Sqm.

core slabs/solid cast in situ slabs, parapets, block wall, plaster, floor and ceiling finishes,

Conveying system: 8 nos. passenger lifts + 1 no. of service lift

Electrical Works:
 Supply and installation of light fittings, wiring, cable trays, final DB’s, SMDB’s, LV Panels,
Fire Alarm System, Miscellaneous low current system etc.
Mechanical Works:
 Supply and installation of chilled water AC System for the complete facility, fire fighting
system, water and drainage piping including sanitary fittings, etc….
External Works:
 Construction of underground water tank, pavement works, curbstone, interlocking paving
tiles and civil works relevant to MEP works, landscaping and irrigation works.

Amana Contracting & Steel Building Company is committed to provide and maintain a healthy and

activities.
Amana Contracting & Steel Building Company recognizes the fact that good health and safety
management has positive benefits to the organization. That commitment to a high level of safety
makes good business sense. It also recognizes that health and safety is an essential function, and
must therefore continually improve.
The success of this statement and the implementation relies on the commitment of Amana Group
Senior Management and it applies to all premises and activities within the control of Amana
Contracting & Steel Building Company.
In order to achieve this aim; Amana Contracting & Steel Buildings Company has the following
objectives:
 To ensure the health, safety, security and welfare of all its employees whilst at work;
 To ensure that visitors and general public, who may be affected by Amana Contracting &
Steel Building Company activities, are not exposed to risk of their health and safety;
 To identify hazards (the potential for harm), assess the risks (the likelihood of that harm
being realized) and manage those risks;
 To consult with employees and ensure all who work on our behalf are adequately informed
of identified risks and where appropriate receive the relevant information, instruction,
training and supervision;
 To maintain work equipment in a safe condition and both safe systems of work and safe
working environment for employees and all who work in our behalf; to maintain
arrangements for ensuring safe use, handling, storage and transport of articles and
substances;
 To maintain clear procedures for action to be taken in the event of an emergency;
 To encourage the development of a positive attitude towards health and safety throughout
Amana Contracting & Steel Building Company.
 This Health and Safety Statement of intent will be reviewed and, if necessary, revised on an
annual basis.

AMANA ENVIRONMENTAL POLICY STATEMENT

This statement has been developed by Amana Contracting & Steel Building Company to stress
our beliefs that successful Environmental Management plays a significant role in its business
and to confirm to our commitment to the continuous improvement in this area. For this reason it
is Amana Contracting & Steel Building aim, that no harm should come to the Environment and
that adequate protection is provided. It is recognized that Amana Contracting & Steel Building
Company and all work produced on our behalf will meet and where necessary exceed all
relevant Environmental Legislation, Regulations and other applicable requirements.

 Control all environmental impacts based on the prevention of pollution or, where this is
not possible or practicable, to minimized pollution.
 Are aware of any risks to the Environment which may occur within or outside the agreed
site area as a result of their activities.
 Seek advice from the approved specialist environmental consultants as to what measures
should be taken to prevent or reduce to an acceptable level (as agreed by the local
authorities) the impact on the environment on the activities taking place under the
contract in question.
 Provide information, Training and Instruction to enhance awareness of relevant
environmental issues and ensure effective management of environmental impacts.
 This Environmental Statement of intent will be reviewed, if necessary, revised on annual basis.

3.2 Project Safety Plan Introduction

This plan for this project “The Construction, completion and maintenance of
“Schlumberger Accommodation MLC – MEA” - Abu Dhabi, UAE” is developed to
regulate the working conditions of all personnel involved in Construction activities, @ site.
It shall govern all safety aspects of all personnel, including sub-contractors’,
Consultant’s, full-time and part-time personnel, employees and guests involved, at any
stage, in the construction activity on site.

The purpose of this plan is to document the Contractor’s HSE Policy, instruct and
guide employees whose actions affect safety in the course of their day-to-day
activities, so as to achieve the company’s overall objective of safe and accident-free
working environment at site.

3.3 Scope and Objectives of Project Safety Plan

3.3.1 Scope
The scope of the plan covers the site activities of all Amana Staff and
operatives, subcontractors, suppliers and visitors. It also extends to the duty of
care required to ensure that those members of the consultant’s team and other
contractor’s staff including the general public are protected from risks and
hazards arising from the building process.
This Safety Plan is prepared in line with the Company safety Procedures and
other Local Authorities Requirements.

3.3.3 Safety Objectives


 Zero Fatalities : 0-1 LTA/1 Million Man-hours Exposure
 Frequency Rate : 0-1.5 LTA/1 Million Man-hours
 Severity Rate : 0-2 lost days/1 Million Man-hours
 No Explosion/Serious Fire/Pollution
 No Permanent Disability Accident
 Report as many near misses as possible: greater than/month

3.3.4 Safety Targets


The main Safety Target and primary obligation in any situation is the
prevention of accident and preservation of life.
The other safety targets of this HSE Plan:
 The protection of environment and property.
 Maintaining good communication and liaison with COMPANY to ensure that
all concerned are, at all times, fully informed.
 The provision for measure to be taken by the various works on site as well by
the staff at AMANA office to ensure a complete coordination between both
parties.
 The most efficient use of equipment, materials and services of all means
involved.

3.3.5 Persons responsible for the implementation of this PSP:

4.0 SITE SAFETY AND ENVIRONMENTAL MANAGEMENT

4.1 Safety Committee

 The project will have Safety Committee chaired by the Project Manager with
members made up of all site sections heads and subcontractors representatives.

 On a weekly basis, the safety committee will carry out a safety walk once a
week in all the areas of work to record their observations. The day and time will
be fixed in agreement with the Project management. The safety problems
identified and action required to be taken.
 The safety committee shall convene to discuss the major safety issues and
project safety performance. Every reasonable effort will be made in the
elimination or control of all accidents, fire, health and occupational hazards
associated with the company’s activities and to maintain the health of
employees in preventing injury and exposure of illness.
 The Safety Committee is in charge of reviews of accident, major safety
violation and render decisions which shall be final, copies of the decision shall
be furnished to each member of the committee.

The Safety Committee might be then formed by:


o Client and Consultants’ Representatives
o Project/Construction Manager
o QHSE Manager
o Safety Officer
o General Foreman
o Representative of Major Subcontractors

The Safety Committee might then meet to review:

o Conformance of the HSE system to the company’s policy & objectives.


o Incidents & incident closing reports
o Near misses
o Emergency & emergency responses
o Any other item relating to HSE system.
o Safety audit report and corrective/ preventive actions on the non-conformities.
4.2 Management Responsibility

This HSE Plan is setup as per the HSE Policy defined by Amana Cont. & Steel Bldg.
Co. It follows, in particular, the “HSE Policy Statement” defined and approved by the
Project Manager – please, refer to clause 1.0 here above.

The contents of the HSE Policy are disseminated throughout the company (Amana
Cont. & Steel Bldg. Co.). Briefing of all new staff on the HSE policy and the
requirements of the company’s HSE system will be carried out.

In principle, Safety is everyone’s concern. As such, Site engineers and supervisors are
responsible for directing and coordinating the management activities of the works, in
line with the established HSE procedures. In particular, the responsibilities of the
personnel are listed hereafter.

4.3.1 Project Manager (PM)

o Responsible for the total compliance and implementation of this HSE plan and
held accountable for the projects safety performance.
o Review and approve job procedures and risk assessment.
o Establish a good house keeping standard and good operational practice.
o Monitor the implementation of this HSE plan on a day-to-day basis in close
coordination of the Safety Officer.
o Chair the site safety meeting.
o Provide the necessary resources required for implementing this HSE plan.
o Display his commitment to Health and Safety, and address the site safety issues
on daily basis.
o Recognize good safety performance from the individuals and team.
o Ensure that accidents and near misses are investigated and reported
appropriately.
o Seek advice and assistance from the HSE department and keep abreast of
changes in legal requirements and best practice.

4.3.2 Site Engineers/ Supervisors/ Foreman

o Understanding this HSE plan fully and following the same in their day-to-day
activities.
o Ensure that all personnel undergo HSE induction training before they are deployed at
the site.
o Enforce adherence to safe job procedures and risk assessment controls.
o Identify weaknesses and deficiencies in job procedures, rectify these if possible and
where ever necessary refer these to the Project Manager/ Safety Officer.
o Incase of serious accidents or incidents, take part in the investigation team and agree
remedial measures.
o Ensure that all employees working under them implement the relevant requirements
of this HSE plan.

o Giving safety instructions to their workmen on a daily basis as a part of the job
instructions, highlighting the possible hazards of the day’s work and the precautions
to be taken.
o Ensuring that the operatives working with them are issued with necessary PPE and
they use the same.
o Keeping their work area neat and clean, especially at heights free from loose
materials.
o Promote the continuous improvement of HSE performance.
o Verifying the third party test certificates while hiring cranes or any lifting appliances
for their validity.
o Organizing and conducting toolbox talks to their workmen every week.
o Identifying and eliminating all unsafe conditions in their respective work area by
carrying out safety inspection everyday.
o Taking suitable corrective measures to rectify the unsafe observations pointed out by
the safety advisor in his inspection reports.

4.3.3 Safety Officer

The Safety Officer will be responsible for implementing and maintaining the project HSE
Plan in full conformance to Employer’s requirements. He reports directly to the PM, for
review and as a basis for improvement of the system’s performance. He is responsible to
ensure all site activities are covered by Safety Inspections and will initiate and follow up all
required corrective actions.

His responsibilities, in brief, are:


o Monitoring implementation of Project Safety Plan.
o Managing the day-to-day operations of the HSE system, as required by
procedures.
o Imparting adequate training and awareness of HSE systems and regulations,
especially safety for all personnel on site.
o Ensuring emergency procedures are prominently displayed and known.
o Ensuring First Aid facilities are available on site.
o Ensuring all personnel is wearing personal protective equipment, as per
regulations.
o Monitoring safety complaints and following up with site team for close out.
o Ensuring preventive actions are taken to avoid repetitions of non-
conformances.
o Interaction with PM for monitoring safety performance.

o Prompt reporting of all accidents on site and investigation and assistance


during investigation.
o Carrying out safety inspections and audits on site, recording and reporting
status and effectiveness of the system.

The Safety Officer is authorized to:

o Interact and agree on all safety issues with the PM.


o Stop any hazardous activity not complying with safety regulations.
o Report on safety violators.

4.2.4 Storekeeper/ Material Coordinator

o Ensuring a minimum stock level of all safety appliances at stores.


o Issuing safety appliances on a daily basis to the technicians as authorized by
the site engineers, supervisors and safety officer, and maintaining records of
the same.
o Obtaining test certificates while hiring cranes and while receiving any lifting
tools and tackles and maintaining records of the same.
o Ensure that proper house keeping is maintained in site stores.
o Ensure that proper arrangements are made for the storage of highly flammable
material. Appropriate warning and safety signs will be ensured at such places.
o Ensure and maintain proper fire fighting equipment in store area.
o He shall arrange to remove all scrap and waste material from site regularly.

4.2.5 Workers

o All workers shall follow the site HSE rules and regulations.
o All workers shall use protective devices and clothing provided to protect them
from dangers of accidents and of occupational disease.
o They shall not tamper with any of the safety arrangements made at site.
o They shall refrain from creating any hazards and from performing any act that
may hinder execution of these instructions.
o All workers shall be responsible for the implementation of the relevant
requirements of this HSE plan.
o Be familiar and show commitment to the implementation of this HSE plan.
o Alert their Manager, Engineer, Supervisor and Safety Officer if any hazards are
observed in their place of work.

o No workers shall carry out any work that will put themselves or others in
danger at any cost.
5.0 SITE START - UP

5.1 Site Start – up Safety Procedure

5.1.1 Legislative Compliance

Amana Contracting & Steel Building Company shall comply with all relevant
federal and National Legislation as a minimum this includes
o Ministerial Order No. 32, 1982
o Abu Dhabi Municipality and Town Planning, Building Directorate, Health and
Safety at Work Manual – Part 2
o Federal Law No. 24 of 1999 for the Protection and Development of
Environment
o Federal Law No. 21 of 2005 Regarding Waste Management in Abu Dhabi
o Decree No. 37 of 2001 Executive Guidelines of Federal Law 24 of 1999
concerning Environmental Protection and Development
o Other approved International Standards may be applied in order to achieve this
practice.

5.1.2 Site Access Control & Site Security


o Amana Contracting & Steel Building Company will provide Security
Personnel round the clock who will be in – charge for the day to day security
of the site. Unauthorized personnel will not be allowed to enter to the site.
o Assigned Site Office Clerk and Storekeeper will ensure that after the
completion of work, the site office and stores will be locked and lights are
switch off.
5.1.3 Site Traffic Management & Road Signs

o General Access routes will be clearly designated and marked. Security


Personnel will assign to control the traffic movement and ensure that the
general access route is always clear.
o Pedestrian Routes will be clearly marked and isolated. Appropriate sign
boards will be in place and Security and Assistant Safety Officer will monitor
it.
o All construction machinery will be equipped with reverse horn and banksman
shall be assigned to guide the reverse movement.

o The maximum allowable speed limit of a vehicle on the construction site may
vary in each emirate. However when not stated, 15 – 20 Kph may be adapted.
o Speed limit signboards will be displayed at appropriate locations visible by
the drivers and operators.
5.1.4 Allocation of Space & Control of Subcontractor’s Site Facilities
o It shall be in the condition of each contract that no subcontractor that no
subcontractor shall require any worker to work in surroundings or under
working conditions, which are, hazardous or dangerous to his health and
safety.
o It shall be ensure that prior to any subcontractor commencing work on site;
subcontractor shall be made aware of Amana HSE Policy and of their
responsibilities towards HSE issues.

5.1.5 Organization of Site Crane and Hoisting Facilities


o Crane operation shall be under the control of trained banks man. Rules for
crane and hoist signaling shall be followed by persons operating/guiding
cranes and hoisting machines.
o Warning signs, signals and barricades will be provided at the work area. On
high ways or streets, flagman or other appropriate traffic control shall be
provided.
o High visibility vest shall be worn by banks man/flagman.

5.1.6 Environment, Housekeeping & Site Tidiness


o Rubbish materials will be collected, stored and disposed in the site skip bins.
o A separate housekeeping team will maintain the in an orderly condition.
o Dust will be controlled by spraying water and noise will be controlled by
isolating the machinery/generators. Other means as described in the Clients
Environmental site controls shall be applied.
o Hazardous materials will be stored/disposed base on the information
contained in Material Safety Data Sheet (MSDS) for the substance.
o Fuel Storage tanks shall be bounded.

5.1.7 Provision of Electrical & Water Supply to be used for the project
o On the award of the contract, the PM shall advise the Garage Manager who
will arrange generators or supply by the Power Supply Authority and Water
Supply to arrange for the temporary power interim, for the provision of supply
and to give them advice on the system required for temporary use or by other
means water will be delivered in daily basis.

o Regular inspection will be done to ensure that wiring, breakers, switches,


plugs, etc.. Are being maintained and used in safe and sufficient manner.
Every temporary electrical installation shall be under the control of competent
person (PM/Electrical Engineer) who will accept responsibility for its safety
and use.
o A No – Objection Certificate (NOC) from the service providers shall be
obtained and retained at the site prior to excavation of underground services.
o If in doubt about buried services, a detector must be used to confirm the
location.

5.2 Health & Welfare

o An adequate supply of potable water and place of meal shall be provided in


project sites. Water containers must be tightly closed and equipped with a
tap.
o Adequate and suitable personal hygiene (washing) facilities, eye wash
station, First Aid Room and toilets with sufficient ventilation

5.3 Site Planning

o This section outlines the plan to be followed prior to and during a contract. It
cannot claim to cover type of contract that is likely to occur, but it does provide a
detailed logical process to serve as a guide in deciding upon a plan of action.

5.3.1 Initial Planning

o Prior to the start of any work, an analysis is made by the responsible to ensure that
the project will be profitable. Similarly, accident prevention is analyzed, both at the
planning stage and throughout the contract, so that the workers will be able to
eliminate or reduce accidents. Accident prevention is a real factor in the economic
success of all contracts.
o Among other factors that must be considered at the initial planning stages are;
protection of employees and equipment, transportation, lifting equipment,
demolition, excavations, and scaffolding.

It is AMANA requirement that employees must be transported only in the passenger


compartment of vehicles. The provision of buses for larger sites will have to be considered.
Inspection and maintenance of vehicles and the selection and training of drivers are other
major considerations. Observation and application of speed limits by all drivers is a priority.
o Access to the site will not be allowed.
o No vehicles will be allowed to enter the construction site with out permission.
o All vehicles to have audible/visual warnings while reversing.
o All the vehicles will be operated by drivers holding valid license.
o No vehicles shall be allowed to reverse on the crowded work premises.
o It will be the responsibility of the drivers to ensure that the vehicles are not
overloaded beyond its rated load.
o They will also be responsible for informing the Transport in-charge whenever the
regular maintenance of the vehicle falls due.
o Drivers and operators will not be allowed to work more that 12 hours continuously
on any circumstances.
o Vehicles will only park at the designated locations in the site without obstructing
the free flow of the traffic.
o The materials will be tied properly when loaded in the vehicles to prevent its
dislodgement while moving.
o Vehicles speed and movement will be controlled at site.
o Proper area shall be marked for collection and dropping

5.3.3. Lifting Equipment

o Cranes, hoists, and lifting equipment should be considered well ahead of actual
requirements both from the point of view of economical use and the safety of
personnel and equipment. All of them should be certified by the concerned
inspection authorities.

5.3.4 Excavations
o Timber for shoring, steel sheet piles, etc (in case sand is loose) should be available
on site prior to starting excavation work. Close supervision of excavation work by
men experienced in that type of work is a necessity.
o For all excavation work, the following guidelines should be followed to achieve
and maintain the safe working place:
 No one should enter an excavation unless it has been inspected and considered
safe to do so by the supervisor.
 Ensure that there is a safe means of access and egress to and from the
excavation.

 When excavating, warning signs and barricades must be provided to all


trenches, pits and wells.
 Employees working in an excavation should be provided with the necessary
protective equipment.
 Where “live” services are present, hand excavation must be carried out until the
services has been identified and made safe.
 No person may work in an excavation. Danger resulting from a collapse of the
sides of the sides of the excavation may be eminent. In such circumstances,
suitable and sufficient shoring will be provided to ensure safe working, or the
other sides battered back to a safe angle.
 During the hours of darkness any excavations, openings or obstructions near or
on the roadways and walkways must be barricaded and protected in accordance
with the excavation procedure.
 Materials, equipment and spoil heaps from excavation should be kept away
from the edge.

5.3.5 Excavators
When carrying out work associated with excavators, you can be subject to hazards which
arise with out any warning.
o Following Rules should be strictly observed.
 Remember that, with modern hydraulic machines, all motions – dig, slew,
raise boom, etc. is much faster than non – hydraulic equipment.
 The tail swing and the boom can be sudden and lethal if you are in the way,
stand well clear of both the operating boom and the tail swing.
 Always face excavating machinery – never turn your back on it. This is
particularly important if you are bottoming up in a supported trench and the
excavator is removing the loose materials.
 Excavators are allowed to work as crane only when handling materials
associated with the excavation work. When working the excavator as a crane
make sure that the suitable slings and fixing points are used and the slinging
method has been carried out properly. Check the Safe Working Load of the
machine; it must not be exceeded.
 Never stand under the suspended load.
 Banksman should be always available during operation is going on.

Where work has to be carried out above ground level and where no permanent work place is
available, temporary work places in the form of scaffolding must be provided. An adequate
supply of scaffold material must be on site. There must be sufficient experienced men
capable of erecting and dismantling the scaffolding properly. Ladders, built to an acceptable
standard, must be supplied and readily available for access to elevated work areas.

o Scaffolding shall be erected, moved, dismantled or altered by qualified personnel


working under the supervision of a designated competent person.
o The footing or anchorage for scaffolds shall be sound, rigid and capable of
carrying maximum intended load without settling or displacement. Unstable
objects such as barrels, boxes, loose brick or concrete blocks shall not be used to
support scaffolds or planks.
o A qualified person shall train employees those are required to perform work of
scaffolds.
o Legs or uprights of scaffolds shall be securely and rigidly braced to prevent
swaying and displacement.
o Overhead protection shall be provided for personnel on a scaffold exposed to
overhead hazards.
o Special precautions shall be taken to protect scaffold members when using a heat-
producing element.
o Tools, materials and debris shall not be allowed to accumulate in quantities to
cause a hazard.
o Standard planking shall be of scaffold grades or equivalent as recognized by
approved grading rules for the type of wood used. Toe boards should be at least
3½" tall.
o Standard scaffold planks shall extend over their supports at least 6 inches and not
more than 12 inches. All standard platform planking when overlapped shall be
overlapped a minimum of 12 inches and secured from movement.
o Scaffolds or their components shall be capable of supporting without failure at
least 4 times the maximum intended load.
o Scaffolds shall not be loaded in excess of the working load for which they are
intended.
o Inspect all scaffold components before erecting scaffolds. Never use any scaffold
that is damaged or defective in any way.

o Do not work on scaffolds if your physical condition is such that you feel dizzy or
unsteady.
o Scaffolding components shall be used for the purpose for which it was intended.
o Before climbing up any scaffold, clean up oil or mud build up from work boots to
prevent slipping.
o All scaffold platforms shall have steel tubing handrails 42" high and 21" high
coupled to handrail post, and 3 %" minimum toe boards rigidly secured.
o Scaffolds shall be inspected daily prior to use by a designated competent person.
The competent person shall fill up an inspection tag (scaff-tag) and secure it to the
point of access.
o Obtain approval from concerned electrical representative prior to erecting scaffolds
adjacent to electrical power lines.
o All scaffolds shall have a ladder installed or an equivalent means of safe access to
the working platforms.
o Do not use ladders or make shift devices on top of scaffolds to increase the height.

5.3.7 Riggers
The job of riggers requires through training. The person assigned will be well acquainted
with the capabilities of the crane being used, hand signals, the different functions of lifting
gear, and various methods of loading.

o Rigging equipment must be constructed according to recognized standard.


o The safe working load (SWL) of rigging equipment is the maximum load which
the equipment will be subjected to; this load will never be exceeded.
o Before use, all new equipment will be subjected to a proof load test by the
manufacturer and certified.
o The safe working load (SWL) and serial number will be clearly marked on the
sling and the lifting gear.
o Riggers will not use lifting gear unless the safe working load is clearly visible.
o The slings will undergo shock loading when a load is suddenly lifted or when the
hoisting is abruptly stopped. This can be caused by the sudden application of the
crane brake. A shock load can increase the normal working load by as much as five
times its value.

o Before storage, chain and wire rope slings will be cleaned, lightly lubricated and
inspected. Slings will be stored in location where they are not liable to suffer
mechanical damage, away from extremes of heat, cold, and especially dampness.
o All rigging will be inspected at least every six months per Standards/ requests and
Sling Inspection Report will also be completed and filed for review.

5.3.8 Chain Slings


o Only approved slings complying with material code shall be used.
o The chain will be clearly tagged by the manufacturer, and riggers will be trained to
look for the safe working load marked on each sling.
o Damaged chains will be returned to the manufacturer for repair or destroyed. On-
site welding repairs cannot provide necessary controlled conditions to safely repair
a damaged chain.
o Logger chains or chains used to secure truck loads shall not be used for rigging.

5.3.9 Wire Rope Slings


Wire rope is the most common type of sling in use on construction sites. Wire rope slings
will be manufactured, inspected and load tested by a recognized manufacturer. Home made
wire rope slings are not allowed.

o Damaged slings will be destroyed if there are:


o 10 random broken wires in one lay.
o 4 broken wires in one strand of rope lay.
o One broken wire at the fitting.

5.3.10 Hooks
o Hooks will be fitted with a safety catch on the hook opening, or should be mounted
with wire and a shackle used or the hook should be designed so that the slings
cannot be displaced.
o Load will be applied on the hook only in the part designed to take them (i.e. the
bend (bow) of the hook). Point loading can result in over stressing the hook
causing it to open or break. Therefore, point loading will not be permitted.
o Hooks will be regularly inspected for signs of damage.
o The hook shall be removed from service for the following reasons:
o If there are visible cracks.
o If it is twisted 10 degree out of place.

o If there is opening through opening beyond the manufacturer’s specification.

5.3.11 Spreader Bars


o All spreader bars will be manufactured, tested and inspected.
o Spreader bars will be permanently identified with the safe working load,
manufacturer’s name and serial number.
o Spreader bars will be stored away from moisture, and protected from physical
damage.
5.3.12 Shackles
o Shackles are used for making connection in rigging. They will be tested by the
manufacturer and marked with the safe working load.
o The pins are separate but matched parts of the shackles, so care must be taken to
use the correct pin for each shackle. Rebar, mild steel bolts or similar items are not
acceptable replacements for shackle pins.

5.3.13 Site Layout


The site layout must be planned before any work is started. This will assist in making the
job easier and more efficient, thus increasing productivity and profit.

Items to be considered under site layout:


o Site temporary offices, toilets, stores, fueling area, rest rooms etc.
o Adequate access roads - to be kept clear and unobstructed all the time.
o Safe means of access and egress - to be provided on site wherever employees are
likely to go must be kept clear and unobstructed at all times.
o Parking facilities - to be provided on site.
o Drainage - The site shall be prepared and arranged such that it is free and clear from
run off of rain water and any other water sources or drainage that might occur
during construction.
o Sand - In special situations shall be given to the movement or build up of sand. This
would be in sand dune areas that might lie adjacent to or around a
construction site.
o Fire Prevention - Suitable fire extinguishers and fire fighting equipments must be
readily available on site. The area around fire extinguishers or hydrants must be
kept clear so that they are readily accessible in case of emergency. They must be
regularly inspected and maintained. Site personnel must be trained in the use of fire
fighting equipment.

5.3.14 Storage Area

o Storage areas must be adequate for all material and equipment to be stored.
o They should not be within 20 feet of permanent or temporary structures.
o Storage areas should be clearly marked.
o They should have directional signs to facilitate speedy delivery.

5.3.15 First Aid

o Male Nurse/Trained First Aider will be made available at site.


o They will be identified with proper stickers on their hard hat and telephone number
will be posted at prominent location.
o Injured persons at site will be evacuated with the help of the facilities available at
site.
o First Aid boxes will be made available at site office and stores where workers can
have an easy approach.
o All major injury cases will be taken to the Hospital after necessary first aid.
o Ambulance facilities of the Police Department will be called in to handle major
injury cases.
o All First Aid cases will be recorded.

5.3.16 Good Housekeeping

The required standard of housekeeping must be established and all site personnel informed
of this standard. The standard must be enforced and maintained at all times.
Housekeeping is an important element of accident prevention. It should be the primary
concern of all supervisors and till the end of the job they shall monitor it.
The following will be ensured:
o Proper access and egress shall be maintained at all times at site and it shall not be
blocked.
o Enough lighting shall be ensured at the work areas. Task lights shall be provided
wherever necessary.
o Trash containers will be kept near the rest area and workers will be advised to
dispose food waste only in trash containers.
o Smoking will not be permitted in the entire site, including site stores. Separate
smoking area will be designated.

o All materials will be neatly stacked in the assigned location provided with suitable
enclosures.
o Packing materials, if any, shall be removed from the site immediately after opening
the boxes.
o Scrap generated at site will be removed in regular basis. The concerned supervisors
will be responsible for ensuring this.
o Welding cable, power cables etc. will be laid in such a way that it will not cause trip
hazard.

5.3.17 Construction sites & Materials Storage in or near Residential Areas

o Engineer shall establish the perimeter of the site by agreement with the proponent
Department. He may consult the HSE Division for assistance.
o The perimeter fence shall be of sound construction and of a design appropriate for
the intended protection of the area and property grounded.
o The fence shall be frequently inspected by a representative of the Construction
Engineer.
o Any access paths through the proposed site shall be re-routed outside the perimeter.
o A suitable number of access gates shall be provided and be of sound construction.
The number of access gates to be kept to a minimum on main thoroughfares.
o Stop signs are to be fixed to the inside of the vehicle access gates.
o Temporary signs should be erected to route traffic in the safest manner to, from and
throughout the site.
o All power driven construction equipment must have the ignition locked and key
removed. If there is no lock for the cab and access can be readily made in to the
engine compartment then the machine must be made immobile by disconnecting the
battery or by any other means especially if the equipment is left outside the
perimeter. This must be carried out before leaving the equipment unattended.
o Crane booms, bulldozer blades and front-end loader buckets should be lowered
when the machine is left overnight. Where this is not possible with a crane boom
then the boom will be locked off in such a manner that it cannot be dropped or
blown with the wind.
o The electrical power to be de-energized over night unless is required for specific
lighting facilities or use by a night - watchman.

o The project will provide its own barricades


o Any excavation or obstruction of any kind likely to create a hazard to vehicular
traffic at night will be adequately lit and sign posted.
o There shall be no indiscriminate tipping of excavation spoil or building materials.
Fire hydrants, manholes, electrical conduits will be kept clear at all times. The
Construction Engineer shall designate sites for tipping.
o All manhole covers will be replaced or the manhole barricaded off.
o Guide ropes will be clearly marked day and night as will any rope used as a
barricade.
o Refuse from the job site must be removed on a regular basis. Any refuse outside the
perimeter must be removed on a daily basis.

5.3.18 Disposal of Waste Materials

o All the scrap and waste materials generated at site will be placed at the scrap or skip
bins provided.
o This will be the responsibility of the respective group of workmen and the concerned
supervisor to ensure the same.
o The garbage bins provided will be cleared regularly. The storekeeper will be
responsible to ensure this.
o A skip will be maintained at site if found necessary to dispose all the solid waste
materials and it will be removed through authorized agencies at regular
intervals.
o Hazardous waste materials, empty buckets of paints, thinners and chemicals, sharp
metal off cuts etc. to be made safe prior to disposal.
o The Project Manager will allocate separate crew under a foreman for scrap and waste
removal if he feels it is required.
o At no stage scrap or waste material will be allowed to accumulate in construction
area.
o The storekeeper will be responsible to arrange to clear the bins regularly.

5.3.19 Personal Protective Equipment (PPE)

o The company will issue the require PPE to all workmen employed by the company,
depending on their work activities. All workmen employed at sites will be required
to use Safety Shoes, Safety Helmets and Coveralls (on which name of the Company
logo will be prominently displayed). It will be the responsibility of the respective
supervisor/ site engineer to ensure that the operatives work under them are issued
with necessary PPE.

5.3.20 Barricades/ Signs/ Tags/ Posters

o All Hazardous areas of operation will be identified as the work proceeds and it will
be barricaded. Necessary precautions will be taken to warn others about the hazards
present.
o Hazardous work areas will be barricaded.
o If the edge protection or barricade is removed to gain access to the prohibited
locations to carry out any specific work there, it will be carried out with permission
only. The removed protection shall be replaced after the work is completed.
o Information posters will be put up in appropriate locations indicating the emergency
telephone numbers and assembly points.
o No smoking signs will be installed wherever required.
o Emergency procedure will be displayed at appropriate places.

5.3.21 Fire Prevention on Site

o The project will be a no smoking site. “NO SMOKING” sign will be posted at
appropriate places.
o Access will be restricted to site work areas, especially out of working hours.
Smoking restrictions will be enforced, adequately notified to all and signed,
where flammable materials are likely to be present.
o Emergency procedure of the main contractor will be followed in case of a fire
accident.

Safety Meeting will be conducted in regular basis and the Project Manager shall chair the
meeting together with the Safety Committee.

Objective:
o To review the implementation of the plan.
o To create a safe environment, to review the HSE performance on site, to ensure all
work activities are carried out in a safe manner.
o To find any solutions for any safety issues remaining unresolved.
o To identify the problem in some areas and to work out suggestions for the same,
o To brief on the accidents occurred previously, if any, to communicate lessons
learned.
o To decide any special safety requirements to be taken in all activities in future.
Safety officer will be responsible to record the minutes and follow up the
implementation of the actions agreed. The minutes will be circulated and will also
maintain at site for audit purposes.

5.3.23 Work Control Measures

In a construction site there is a need for a written safe system of work in a specific
circumstance. A Permit to Work System should form part of that “safe system”. Permit to
Work must only be issued by an appointed authorized person, using duplicate and numbered
forms. They must be signed by an authorized person and by a competent person who will
carry out (or be responsible for) the work described on the permit.
Permit to Work System are essential to ensure safe working and freedom from hazards
where high-voltage electrical supplies, cables and equipment exist, particularly in
maintenance or construction work. Entry to such area will be strictly controlled.

Hot Work Permit – for work involving the use or exposure to a source of ignition.
Before starting any Hot Work, check the space for any flammable or explosive mixtures and
oxygen level.
Cold Permit – for activities such as cold cutting, scaffolding, pressure testing……….
Excavation Permit – for work involving excavation on site and off – site projects.

A weekly HSE Meeting together with Client Representative and Consultants will be
conducted to discuss the major safety issues e.g. review of Project Safety Plan and other
Safety, Health and Environmental matters.

6.2 Daily Site Safety Walk (Inspection)


On daily basis, Site Safety Officer will formally inspect all working areas to identify the
safety problems and bring them to the attention of the concerned supervisor and later
will take the necessary corrective measures.

6.3 Weekly Inspection


On a weekly basis, Safety Officer will carry out a formal site assessment and Inspection
Report will be submitted to the Project Manager.

6.4 Monthly Project Safety Statistic Report


Shall be prepared by Safety Officer and signed by the Project Manager. A copy shall be
sent to the QHSE Manager

6.5 Construction Safety Inspection and Audit


o Project Manager shall conduct a Site HSE Inspection at least once a month. This
may undertake in conjunction with an inspection by the Safety Officer.
o All site supervisors shall be responsible for conducting routine HSE surveillance and
taking appropriate action as they go about their day-to-day work.
o The site Safety Officer will inspect the entire site on a regular basis and will give
inspection report to the Project Manager.
o The respective Engineer, supervisor or the foreman will attend all unsafe conditions
pointed out by the Site Safety Officer. Project Manager will be responsible to ensure
the corrective and preventive action on the inspection reports.
o All site supervisors will inspect their site and brief the employees of the hazards
related to the job.
o Site Safety Officers will ensure that all scaffoldings, ladder and power tools at site
are recorded properly and inspected on a regular basis.
o Site Safety Officer will also conduct periodical surveys to identify the target focus
areas so that the Safety Management of the site is on the right path always.
o Safety Department will carry out monthly safety audit of the site to ensure the

6.6 Ad – hoc Audits

Ad – hoc Audits of activities may be carried out at any time, out with the audits
previously listed.
o The Safety Officer will conduct regular and frequent Inspections of work practices
being carried out on the site. The Safety Officer will also monitor the health and
safety performance of Subcontractor on site by conducting by safety inspection.
o These will be carried out on regular basis with a view to determining the standards of
Site Housekeeping, health and safety procedure, approved documented system and

6.7 External Inspections


o It is further understood and accepted by Amana Contracting & Steel Building
Company that specialized inspection by external bodies may be conducted, e.g.
Engineer, Employer appointed third party; Civil Defense Officer; when any of these
external visits take place, a member of Amana Contracting & Steel Building
Management will be in attendance.

7.0 HAZARDS AND SAFETY PRECAUTIONS

7.1 General

Based on the Hazard Analysis and Risk Assessment, the following “Hazards and
Safety Precautions” have been developed. These control measures apply to general
circumstances. Site specific details have to be incorporated to these procedures before
carrying out any activity.

Utmost care has been exercised while conducting the hazard analysis. This is the data
currently available. Certain additional environmental hazards may be encountered
depending upon the actual field conditions while executing the work.

Depending on the progress of the project, planning schedules such as monthly, weekly
and daily, all critical jobs will be analyzed to identify the hazards likely to be
encountered during the execution and the precautions to be taken will be decided.

Activity specific Risk Assessment for new activities will be prepared by the Site
Safety Officer based on the Method Statement and the Safety Precautions suggested
by him shall be implemented by the Site Engineers while taking up the said activity.

Before taking up all major activities, a detailed study will be carried out and a Method
Statement will be prepared incorporating all the safety requirements by the respective
engineers.

o Placing of temporary buildings will be planned to ensure the stability and
availability of utility services (water, drainage & electricity). Loading and off-
loading of offices will be planned to ensure suitable crane, lifting equipment and
trained personnel are available. Fire extinguishers will be ordered as required prior
to the occupation of the facilities and supplied to site before work starts.
o First aid facilities will be provided as required by the assessments. Toilets, welfare
and hygiene facilities will be arranged suitable for likely numbers of employees
using them and types of undertaken. Safe access will be provided to all temporary
buildings. A competent electrician will fix temporary cable supplies.
o Fire and emergency procedure will be published. Movement of stores will be
planned to minimize manual handling. Facilities will be cleaned at regular
intervals. Heating, lighting and ventilation to be monitored
o Staff will be trained in the management of health and safety on site and the
requirements of the various regulations, especially in regard to control of
subcontractors. Office workers and other operatives will be trained in Emergency
Procedures.
o Skip bins will be provided in the offices, stores and rest areas to facilitate the
disposal of waste materials.

7.2.2 Working at Height

o Edge protection will be erected at all openings or edge where fall of more than 2
meters could occur. Safety harness will be provided for all workers who work at
height (2 meters or more) where no positive fall prevention can be provided.
o Where edge protection is removed for access, or is not practicable, workers
working at or near the edge will wear safety harnesses.
o Where there is likely to be debris falling, chutes or full enclosures will be use to
protect all workers.
o All workers below overhead operations will wear safety helmets.
o All equipment used will be checked to ensure it is in good order, to correct
specification.
o Work will be monitored to ensure that additional precaution and equipment is
taken in to use if edge protection is removed.
o Training and instruction mist be provided to all workers and supervisory staff
involved in the use of life lines and harness and how to inspect and assess PPE of
this type before use.

7.2.3 Use of Ladders

Hazards:
o Fall of person from ladder
o Ladder slipping
o Objects dropped by ladder user

Precautions:
o Tower ladders will be checked to ensure correct length, type and condition before
use.
o Ladders are subject to the plan.
o Ladders work is restricted to light activities and for a short duration.
o The ground base for the ladder use must be firm and level.
o The ladder must be of sufficient length to extend 1.05 meter above the step off
point when in used as access to a scaffold.
o The correct angle of the rest for a ladder is 75 degrees, or a base to height ratio of
1:4.
o Ladders must be secured against slipping, by tying at the top or at the bottom.
o Over-reaching from ladders will not be used in the jobs where it is likely to come
in contact with live electrical parts.
o Ladders with out non-slip base and hinge locks, wherever applicable, will not be
allowed at site.

o Supervisors must checked ladders before using to ensure that workers are not over-
reaching or using two hands to work.
o Damaged ladders will be broken up or removed from the workplace immediately.
o Painted ladders will not be accepted to use.
o All ladders brought to site will be registered and appropriate number will be given
to ensure that they are inspected in regular intervals.
o All workers must be trained in the safe use of ladders and the hazards, which are to
be avoided. This will normally be done at induction training.

7.2.4 Use of Tower & Mobile Scaffolds

Hazards:
o Fall of person

o Collapse of tower
o Overturning of tower

Precautions:
o Only authorized personnel will erect, modify or dismantle scaffolding towers.
o Tower should not be specified for in the vicinity of the overhead power lines.
o Specification of the use of tower scaffolds will take into account the site ground
condition expected, height restrictions and obstruction.
o Towers will be erected by trained personnel in accordance with the relevant standards
and manufacturer’s instructions. Ladders access should be internal and fixed to the
narrowest side. Maximum height to base ratios will not exceeded 3:5:1 inside use and
3:1 external use without ties. Ties will be used in exposed or windy conditions. All
tower platforms will be fully boarded and fitted with toe boards and guardrails. Wheel
will be locked when the tower is in use. Personnel and materials will be removed before
a tower is moved.
o All scaffolding will be inspected before use. After alterations or adverse weather
conditions authorized personnel must inspect scaffolds. Towers over 2m in height must
be inspected every 7 days. Competent personnel will carry out all scaffolds inspection.
Scaffolds will be checked regularly to ensure their correct use.
o Persons erecting and inspecting scaffolding must be adequately trained. Scaffold tag
system will be followed at site.
o Safety harness will be provided for all workers who work at height (2 meters or above)
where no positive fall prevention can be provided.

7.2.5 Use of Portable Electrical Equipment

Hazards:

Precautions:
o All portable electrical equipment will be identified individually and subject to
planned maintenance. Equipment supplied to site will be fit for its purpose with
regard to voltage, power and environmental conditions.
o All equipment to defective would be switch off and reported immediately. Visual
inspection of the equipment will be carried out before use. Leads and extension
cables are to be routed so as to minimize the likelihood of damage and trip to
hazard.
o Only industrial plug and socket holder will be use at site.
o Only equipment operating at 110 volts or 220 volts with ELCB protection will be
permitted on site.
o All will be made aware of the above Policy concerning use of electrical equipment
o The use of electrical equipment will be monitored to ensure the safe use.
o Site management will ensure that only trained, competent person will test, repair
and maintain portable electrical equipment.
o Workers will be trained in the precautions and safe use of portable electrical
equipment.
o Site Safety Officer ensures that all power tools are regularly inspected and records
will be maintained.

7.2.6 Hand Tools

Hazards:
o Eye injury
o Injury to hands, feet and body
o Slipping and falling due the use of worn out tools

Precautions:
o It is to be ensured that the tools issued are fit for the purpose, the environment in
which they will be used and in good condition.
o Tools are required to be suitable for the purpose for which they will be used.

o Eye protection is to be provided and use whenever work is done using cold chisels,
drills, grinders, or other tools where there is risk of flying particles or piece of the
tool breaking off.
o Open bladed knifes, screw drivers and other sharp tools are to be carried and use
so as not to cause injury to the users or others.
o Insulated tools must be used where there is a possibility of live electrical work.
o Site management will monitor hand tools, which can deteriorate with use, to
ensure they are sharpened or replaced as necessary and to ensure the correct tools
are being used properly.
o Workers are to be instructed in the correct method of use and in maintenance
requirements at induction training.
o Storekeeper will inspect all new hand tools before issuing it to site to ensure that
they are in good working condition.

7.2.7 Welding & Flame Cutting

Hazards:
o Hot burns
o Foreign body falling in body
o Electric shock
o Fire
o Fall of cylinders

Precautions:
o Any gas cutting or welding work can be taken up only after obtaining a hot work
permit. Concerned supervisors will be required to obtain the hot work permit from
the concerned Safety Officer and arrange all the precautions mentioned in the
permit before starting the hot work. Original permit will be at site location and a
copy will be kept at the site office.

Welding
o It is the responsibility of the welder and his crew to comply with all the safety
requirements given on the permit before starting the work. If they find any
difficulty in complying with any of the requirements, they will have to report it to
the supervisor concerned and await further instructions from him. The supervisor
has to take up the matter with the Safety Officer and arrive at an alternate method
of complying with the said requirement.
o The welding machine should be provided with body earthing.

o The welding cable shall be laid in such a way not to cause any tripping hazard. It
should not have any joint between. If there is any, it should be provided with the
insulated male or female couplers to ensure the safe connections. The return path
for the welding circuit shall be preferably from the job itself. The welder holder
should have proper insulation and if the insulation is broken during usage, the
holder shall be replaced immediately. Before the welding work is started, it should
be ensured that there are no flammable materials stored or installed nearby. A fire
extinguisher shall be kept at the place of welding. A fire watch shall be posted, if
required. The welder should use all required PPE. It is the responsibility of the
welding crew to ensure that there is no trace of hot flux or any smoldering fire
caused by it at the workplace before leaving the area.

Flame Cutting

o Oxygen and acetylene cylinders will not be allowed inside any confined
spaces. Trolleys with restraints for oxy-acetylene cylinders only will be
permitted at site during hot works. It will be provided with flash back arrestor
during use. Cylinder key will be kept on the cylinder. The supervisor
concerned to ensure that there is no leakage and regulators will be replaced
immediately. The hoses will be laid in such a way not to pose any trip hazard.
The gas cutters will be required to use gloves and goggles. The supervisor or
the foreman planning to take up gas cutting work will have to obtain a hot
work permit from the concerned engineer/safety officer and comply with all
the requirements mentioned therein. Fire extinguisher shall be kept at the place
of work. A safety watch/fire watch will be posted. Before the work is started, it
should be ensured that there are no flammable materials stored or installed
nearby. If there is any, it should be either removed or covered with fire
resistant cloth. The hot flux shall be contained in the same floor; it shall not be
allowed to fall to the next lower level. It is the responsibility of the crew to
ensure that there is no trace of hot flux or any smoldering fire caused by it at
the place of work before leaving the place.

o Since the duct insulation adhesive compounds are highly flammable, it should
be ensured that there is no hot work such as welding, flame cutting or grinding
takes place in the vicinity. As far as possible insulation of pre-fabricated duct
pieces will be done in an isolated location.
o The adhesive container required for the day’s use shall be brought from the
stores and at the end of the shift, the balance quantity shall be returned to the
stores everyday.
o Empty cans will be returned to the site stores immediately. It will not be
allowed to be lying at site.
o The employees will be made aware of the hazards of the adhesive compound.
The insulation applicators will be provided with mask.
o Insulation work shall be carried out only in well-ventilated areas. If work is to
be carried out in a confined space such as basement rooms etc., suitable
ventilation is provided to remove the vapor from the working environment.
o Incandescent lamps or halogen lights will not be used in the insulation area
since the heat generated from it may lead to fire. The light fittings will be kept
away from the place of work.
o They will be provided with disposable gloves and appropriate masks.
o Fire extinguishers will be made available at the place of work.

7.2.9 Storage of Materials

Hazards:
o Injury to the workers
o Injury to visitors or trespassers
o Environmental contamination

Precautions:
o All materials stored in tiers shall be stacked, racked, blocked or otherwise
secured to prevent sliding, falling or collapse. Materials will not be stored
direct on the ground or on concrete floors.
o Aisles and passageways shall be kept clear to provide for the free and safe
movement of the material handling equipment or employees such areas shall
be kept in the good repair.
o Minimum required materials will only supplied to site and it will be temporary.
o The manufacturer’s recommendations will be followed with respect to
temperature and humidity requirements and stacking.
o Palletized loads will not exceed two pallets in height.

o Materials stored inside the buildings under construction shall not be placed
within 3 meters of an exterior wall, which does not extend above the top of the
material stores.
o Compressed gas cylinders will be stored upright.
o Stacks of cylinder objects such as pipes and cable drums will be stabilized
using chocks and wedges.
o Material stack will be limited in height to ensure stability; heights of more than
2 meters will be avoided unless specifically authorized by the project manager.
o Drums and containers will be marked clearly to indicate contents.
o Secure storage will be provided for all substances, to prevent access by
unauthorized persons.
o Stockpiles and storage areas will be inspected regularly to ensure stability and
to ensure that the above physical precautions are in place. Verbal instructions
and training will be given to the workers as necessary to ensure good
housekeeping standards are maintained on site.

7.2.10 Material handling

7.2.10.1 Manual Handling

o Where possible, mechanical lifting equipment shall be used.
o No employees shall be asked to carry loads above his capacity and in any case
no load shall exceed 50kg.

7.2.10.2 Mechanical Handling

o Before carrying out heavy erections such as motors, panels, chillers, Air
Handling Unit’s etc., a detailed method statement is prepared and a risk
assessment is carried out.
o All lifting appliances like slings, chains, pulley block etc. brought to the site
will be provided with a test certificate.
o The engineer in charge, who would be using the hired cranes, is responsible to
inform the Safety Officer about the job to be performed and it is the joint
responsibility of both to ensure that the hired crane is suitable for the purpose
of the job and is safe to use.
o While transporting the materials in vehicles, it will be ensured that undue
projections from the vehicles will not be allowed. If there is any projection, the
person responsible for the loading operation will ensure that either red flag or
red lamp is tied to the projections to warn the vehicles coming behind.
o Trained and license operators/drivers will only be allowed to operate lifting
machinery.
o All workers will be instructed on the correct and safe way to operate lifting
machinery.

7.2.11 Electrical Work

o Electrical burns

Precautions:
o All circuits to be worked on will be treated as live until verified dead. There is
no exception to this requirement; experience of employees is irrelevant.

o Before any operation or works at site commences and during progress, the
contractor shall take all practicable steps to prevent danger to persons
employed from any live electric cable or apparatus.
o Whenever possible live work is to be avoided. Whenever live work is required
a safe system of work is to be devise; preferably written and by use of a Permit
to Work System.
o Necessary protective devices such as earth leakage circuit breakers shall be
provided in the circuit to avoid risk of shock, fire or burns.
o Access to live conductors is to be controlled and appropriate signs are to be in
place. Written information and instructions will be required for work on
complex systems (control, metering and parallel circuits). A clear access of 1
meter, gloves and matting are to be provided for live working. Electrical test
equipment will be insulated, fused and calibrated. Electricity supply authority
seals will not be broken and final connections will not be given without written
authority.
o Live work is only to be carried out by authorized competent electricians under
direct supervision of the nominated supervisors. Electricians will not Pagebe

permitted to work accompanied on live connections unless specifically
authorized to do and good communication are in place.
o Adequate PPE, first aid and qualified first aider are to be available at the
workplace where live work is to be done. If live work is not covered by a
Permit System, then switching off must be accompanied by a physical lock-off
and appropriate signage.
o Electrical section in charge to ensure that only qualified and competent
persons carry out electrical work.

7.2.12 Cable Pulling

Precautions:
o A pre start survey will be carried out to check the route, manpower and
equipment requirements and the predictable hazards. The work will be planned
to ensure a safe means of access is provided.

o Personal Protective Equipment will be provided to all workers as per


requirements. Good communication must be maintained between levels when
cable is pulled.
o Worked will be monitored to ensure that additional precaution and equipment
is taken into use if edge protection is removed at any place. Close supervision
is required to ensure that over-stressing of the cable does not occur. Cable pull
will be smooth and maintained at a reasonable rate to ensure control and
tension requirements are met. Supervisor to ensure that appropriate signage
and warning signs are displayed.
o Workers will be trained in Manual Handling techniques, and instructed in the
use and precautions required for the safe use of the necessary tools and
equipment.

7.2.13 Steel Erection

Precautions:
Skeleton steel construction
o During construction a safety railing of wire rope shall be installed around the
periphery of all temporary planked or temporary metal decked floors.
Structural assembly
o During the placing of structural members, the load shall not be released from
the hoisting line until the members are secured with not less than two bolts.
o Tag lines should be used for controlling loads.
o Employees shall be provided with fall protection equipment (full body harness)
and instructed them to use.

Bolting and riveting


o Containers will be provided for storing or carrying rivets, bolts and drift pins
and will be secured against falling when aloft. When bolts or drift pins are
being knocked-out, means shall be provided to keep them from falling.

o Impact wrenches shall be provided with a locking device for retaining the
socket.
o Riveting shall not be done in the vicinity of combustible materials unless
precautions are taken to prevent fire.
o When rivet head over knocked off or backed out, means shall be provided to
keep them from falling.
o A safety wire shall be properly installed on the snap and on the handle of the
riveting hammer and shall be used them all the times.

Fitting and plumbing


o Connections of the equipment used in plumbing-up will be properly secured.
o Metal decking of sufficient strength shall be laid tight and secured to prevent
movement.
o All unused opening in floors, temporary/permanent, shall be completely
planked over/guarded.
o Employees shall be provided with fall protection (safety harnesses) and
instructed them to use.

7.2.14 Work in Confined Spaces

Under the Confined Spaces regulations, a confined space can be either

o A place which is substantially, though not always entirely, enclosed or


o A place where there is no reasonably foreseeable risk of serious injury from
hazardous substances or conditions within the space or nearby.

Some Confined Spaces are easy to identify, such as closed tank, vessels and sewers.
Others such as open-topped tanks become confined spaces during their manufacture.
Some places may become a confine space only occasionally, perhaps due to the type
of work to be undertaken, such as room during paint spraying.

Precautions:
o Eliminate need for entry or use of hazardous materials by selection of
alternative methods of work or materials.
o Assessment of ventilation available and possible local exhaust ventilation
requirements, potential presence of hazardous gases/atmosphere process by
products, need for hygiene/welfare facility.
o Permit to Work System will apply in a Confined Spaces. Adequate ventilation
will be present or arranged. The area will be tested before entry and
continually during the presence of persons in the confined space. Emergency
breathing apparatus, rescue harness, lifting equipment and safety line shall be
provided when necessary.
o A communication system with those in the confined space must be established.
o Necessary PPE and hygiene facilities will be provided for those entering on a
confined space.
o Site management will be responsible to decide on the nature of confined space
and to put a safe system into operation. Flood potential and isolation must be
checked.
o Specific Method Statement and Risk Assessment will be prepared before
allowing such works.

7.2.15 Work in and with Excavations

Hazards:
o Collapse of sides
o Striking existing services
o Persons falling into excavation
o Plant and materials falling into excavation
o Flooding of excavations
o Presence of hazardous atmospheres
o Presence of contaminated soil

Precautions:
o Sufficient numbers of trained workers and competent supervisors must be
available before work starts. Sufficient and suitable plant must be available for
trench support before work starts. Suitable monitoring of equipment and
personnel trained in its use will be required where known exposure to toxic
substances or lack of oxygen may occur. Location of existing services must be
complete before work starts, also information obtained on ground conditions.

o Sides of excavations greater than 1.2 m in depth must be supported in case of


loose sand. Where flooding risk exists, water pumps with suitable capacity will
be installed. Substantial barriers will be erected around excavations greater
than 2 meters deep.
o Stop barriers will be used to prevent vehicle entry. Spoil and materials will be
stacked at least 1.5 meter from the edge of the excavations. Ladders will be
provided for safe access/egress. Cable location devices and local authority
drawings will be used to trace buried services prior to the commencement of
work. Suitable signs and barriers will be provided to warn of the work.
o Supervisors to ensure safe system of work provided, taking account of
prevailing conditions including weather, traffic and existing structures. Provide
suitable PPE as required and ensure its correct use. Ensure personnel selected
are capable and experienced unless under direct supervision.

7.2.16 Working inside Manholes

Some manholes may become a confined space only occasionally, perhaps due to the
type of work to be undertaken or the location of it. In such cases the following safety
precautions shall be followed.

Hazards:
o Poison from toxic gases
o Lack of oxygen
o Excessive heat
o Falling material/person

Precautions:
o Surrounding areas of the manhole has to be barricaded. Proper
signage/warning lights shall be place to give warning. Proper and correct key
shall be provided and use to open/remove manhole covers. Workers to follow
the material handling procedure to prevent any back injury while putting or
removing the manhole covers.
o The manhole has to be kept open for sometime to let the fresh air in. If
required fans shall be used to give fresh air.
o Eliminate entry into manhole if the work can be carried out from outside.
o Necessary PPE and hygiene facilities shall be provide for those entering
sewers.

o Apart from the above, specific method statement and risk assessment will be
prepared before allowing such works. Work notification shall be obtained to
carry out such work whenever necessary.
o Supervisor shall be present during the operation.
o Necessary communication facility shall be made available if required
depending on the nature of work.
o All material and equipment will be removed from work area once the work is
over and all barricade and signage shall be removed.

7.2.17 Pressure Testing

Hazards:
o Flooding of the area
o Bursting of the piping/hose/joints etc.
o Damage of the pressurized system due to the activities nearby

Precautions:
o Pressure testing shall be carries out by competent personnel and supervised by
a qualified and experienced supervisor.
o Area around the pressure testing system shall be barricaded. Caution sign to be
placed to warn others at the appropriate places and once the test is over, the
signs and poster shall be removed. Special care shall be taken near the pipe
joints.
o Only calibrated gauges shall be used for pressure testing.
o Suitable contingency arrangements shall be in place to handle any unexpected
leaks at unexpected locations during the course of the pressure testing.
o No repairs shall be tried on pipes under pressure.
o Tool box talks to be given the workers carrying out pressure testing.
o The lines under pressure testing shall not be left unattended at any time.

o The First Aid should be provided immediately in any case of accident involving
personnel injuries.
o Any employee who is injured due to accident on work (no matter how the seriousness
of the injuries), his Supervisor or Foreman should be informed immediately about the
accident.
o Transportation of the injured to the Infirmary or to the nearest hospital should be
arranged.
o AMANA must immediately report the matter in detail to both Police Department and
labor Department, if a worker is killed or gravely injured or becomes seriously ill as a
result of his employment and during it.
o The Safety officer should inform the Safety investigator of client immediately in case
of accident, if it has occurred in client's premises.
o It is the duty and responsibility of AMANA Safety Coordinator and the injury's direct
superior to fill an accident declaration and report form as soon as possible.
o The filling of the site injury report form will be under the guidance of the Safety
officer.

8.2 Action in Case of Injury

Any assistance requested by a person in danger, accident or sick in the construction site
has to be done through Emergency means.

The person asking the aid has to:

o Give the exact location of the victim


o Give the most precise information about the victim.

Until the arrival of the ambulance, the authorized First Aiders will render fist aid. Just
the First Aiders only will help the victim, with less possible movement.

8.3. Action of the Supervisor/Foreman

o Guide the ambulance to the location of the victim


o Give or help for the first aid
o Follow the evacuation to Hospital

For any injury for which the victim is sent to the nearest Hospital, the emergency
communication procedures should be followed. If the required person is absent, the
person who replaces him should be contacted.

Immediate action to be taken to prevent re-occurrence of accident generated on the same


cause in the same place.

8.4 Reports and Records

Reports required

An immediate oral report shall be made to the Company Management in the case of:

o All fatal injuries


o All injuries requiring medical attention
o All damages
o All fires

Initial oral reports of such incidents shall be followed by a written report detailing the
circumstances, corrective action taken, and recommended action to prevent a recurrence.

The written report in most cases shall be on the standard form established by the
Company although, in the case of serious accidents, a fully detailed account of the
circumstances, with witnesses, statements and descriptive photographs would be
required. No matter what type of report is used, the information should always contain
pertinent details.

8.5 Accident Investigation

Accident investigation must be conducted in a manner, which will provide facts rather than
faults. The point of such investigations is to prevent recurrence of similar accidents.
The main purposes of an accident investigation are:
o To find the causes so that similar accident may be prevented.
o To determine the point at which "unplanned" events took over from the "planned"
sequence of events.

8.7 Cases to be Investigate

All incidents that result in property damage or serious injuries to personnel must be fully
investigated. Unless the real cause is known, the hazard cannot be controlled in the future.
The ‘near miss’, which does not necessarily produce injury, is equally important from the
point of view of prevention and should also be thoroughly investigated.

8.8 Principles of Accident Investigation

The scene of a serious accident should be left undisturbed until the safety investigator has
had the opportunity to inspect and investigate. The investigation should include but not be
limited to the following:

o Questioning the man in charge and finding out what was planned.
o Find out the injured person or the normal position of the damage equipment or plant.
o Questioning the injured person as soon as possible.
o Questioning the witness as to what they actually saw, not what they think happened.
o Finding the explanation for any irregularities.
o From the information obtained, establishing the reason why the ‘unplanned” took
over from the “planned”.
o Recommending methods of implementing corrective action.

The company has established training plans to make its employees, at each relevant
function and level, aware of:

The training needs of personnel will be reviewed at regular intervals. Where training

Importance will also be given to have control on the environmental factors such as
chemical, physical and biological exposures or stresses arising out of the workplace,
which may cause sickness, impaired health or well being, or significant discomfort and
inefficiency among workers.

9.1 Safety Training for Supervisors/ Foreman

The immediate job of preventing accidents falls upon the Supervisor/ Foreman, not because
it has been arbitrarily assigned to him, but because accident prevention and production
control are closely associated supervisory functions.

The most direct way to develop the desired attitudes and to impart the necessary information
about safety to supervisors/ foreman is to provide a course of instruction.

The course for supervisors should include legal requirements, company and administrative
policies, and safety aspects of the work likely to be undertaken by the supervisors/ foreman.

Hopefully those who receive training at recognized training establishments would have
the entire necessary safe working elements integrated into the basic instruction for their
particular skills.

8.2.2 Specialized safety training applied from within the Company. There are two aspects:

(1) Induction training for new employees of the Company. It should be realized that a new
entrant is likely to be exposed to risk before his job training is complete. It should also
be recognized that both the new entrant and new employee are more susceptible to
accidents in their early working career.

Induction training should familiarize new men with the common types of hazards on site
and the precautions that they are expected to take. It should also cover the broad
requirements of the company's own accident prevention policy and instruct newcomers
specifically in those sections which effect them personally, e.g., if it is company policy
that safety helmets and protective footwear should always be worn, then this should be
made clear and they should be told how and where to obtain them.

(2) Suitable induction courses together with job training containing integrated safety content
will go a long way towards achieving safe working conditions. Site supervision,
however, must ensure that correct job methods are utilized and workmen are
periodically reminded that hazards still exist.

1. Toolbox talks will be conducted at least once in every week by all the site
engineers/ supervisors to their respective workmen before the shift begins.
2. The Project Manager shall issue a schedule of the toolbox talk program. In
addition to this, depending upon the site activities in progress, job specific hazards

The procedure for document control details the methodology for preparation, review,
approval, distribution, amendment and control of obsolete documents. It shall be the
responsibility of the Safety Officer to control the documents relating to safety.

All the HSE system parameters are monitored and records will be maintained as
appropriate to the system to demonstrate conformance to the requirements of the
safety management system.

12.0 OPERATIONAL CONTROL

The operations and activities associated with the identified significant safety and
health risks, in line with the HSE policy and objectives, will be identified.

HSE Procedures will be established to cover situations where their absence could lead
to deviations from the HSE policy.

Special attention will be given to non-routine activities, shift handover and final
testing & commissioning.

A system of work authorization by a designated person will be implemented at the


stage of testing & commissioning and energizing of the permanent installations.
Critical activities and processes in the construction activities, which may create safety
and/or health hazards, will be identified, defined and communicated to the employer.
These are monitored and controlled on a regular basis. The system ensures that these
operations are carried out under controlled conditions. Pre-operational checks, where
relevant, will be incorporated into the construction activities to ensure safety and
communicated to the employer.

12.2 Safety Inspections

Procedures will be established to ensure, all activities throughout construction, are


subject to regular inspections. Safety inspections will be scheduled based on the status
and importance of the activity.

All non-conformities discovered during an audit will be recorded on the safety non-
conformity report. The results of audits, in addition to the PM / PE, will be
communicated to the concerned site engineers, who are responsible for timely
implementation of agreed corrective actions.

Procedure requires that the implementation and effectiveness of corrective action shall
be verified by follow-up inspection.

The results of the safety inspections and the corrective / preventive actions taken shall
be reviewed by the Safety Committee, when required.

12.3 Corrective Action

Concerned personnel are responsible for identification, recording and immediate


reporting of any instances of non-conformity to the HSE system. The HSE system
requires that prompt and effective corrective action be taken in all cases where non-
conformances occur.

The authority and responsibility for review of any non-conformity, near-miss and/or
incident, will be defined in the procedures.

The suitable identification of non-conforming items to prevent unintended use, such as


construction equipment causing a safety and/or health hazard, will be established in
the procedures.

HSE project plan ensure that the reasons for occurrence of non-conformities, near-
misses and/or incidents, are thoroughly studied such that the root cause shall be
determined. Results of such investigations will be recorded.

The purpose of this procedure is to ensure that any non – conformity to the HSE System is suitably
identified and efficient corrective/preventive actions are implemented so that any event that may or
had the potential to lead to an accident is prevented.

SCOPE OF APPLICATION

The procedure shall cover all the activities at site including all subcontractors and visitor which may
have a potential safety hazard.

Definitions:

Non – Conformity: means any deviation from work standards, practices, procedures, regulations,
management system performance etc. that could either directly or indirectly lead to injury or
illnesses, property damage, damage to the workplace, environment, or a combination of these.

Corrective Action: action taken to eliminate the causes of an existing non – conformity, defect or
other undesirable situation in order to avoid reoccurrence.

Preventive Action: action taken to eliminate the causes of a potential Non – conformity, defect or
other undesirable situation in order to avoid reoccurrence.

RESPONSIBILITY FOR APPLICATION

1. The HSE Officer has the primary responsibility for the identification and prevention of
any safety hazard at site.

2. Site Engineers, supervisors and the person executing the work, which may at risk to the
hazard, shall be responsible to comply this procedure.

NON – CONFORMITY ITEMS

1. Periodical safety inspection and Safety Audits shall be carried out by the HSE Officer or
designated person.

2. Any deviation from the work standards, work practices, HSE procedures, regulations,
management system performance which may have safety hazard shall be identified,
disposed, recorded and notified to the concerned.

3. Any equipment installation or tools that are not conforming to the safety procedures and
standards shall be immediately withheld from use. These shall be isolated, suitably
labeled/marked as “Not safe for use”, to prevent inadvertent use.

4. Any work practice which may have the potential to cause a safety hazard, shall be
immediately suspended.

5. Defective items or installation which may have a safety hazard, if any observed by the
personnel executing the job at site shall be immediately reported to the Site Engineer or
HSE Officer. Such complaints shall be recorded and handled as Non – Conformities.

6. Any occurrence, event, accident or situation that had a potential for a safety hazard or
has resulted in injury or loss time shall be handled as Non – Conformity. In the event of
accident, Near Miss, activity that has caused the incident shall be suspended.

7. All non – conformities shall be recorded on the Non – Conformity report and a copy will
be sent to the Site Engineer or to the concerned subcontractor’s representative for action,
notifies all areas affected and request to party responsible to propose and implement
suitable corrective and preventive action.

8. Non – conformances shall be notified to the Project Manager with a copy of the Safety
Non – Conformity Report.

9. Work should not resume unless the corrective and preventive actions are effectively
implemented.

CORRECTIVE / PREVENTIVE ACTION

1. The concerned Department (HSE Department) or the Subcontractor’s representative


shall investigate the Non – Conformities for the purposes of identifying the root causes
of the problem and record the findings on the non – conformity report.

2. Having identified the root cause of the problem, HSE Department or Subcontractor’s
representative shall propose or agree the corrective action to eliminate or close the non –
conformity and suitable preventive actions to avoid reoccurrence. The details shall be
recorded in the non – conformity report

3. HSE Department or subcontractors shall implement the agreed corrective & preventive
action within the agreed time frame.

4. The HSE officer shall monitor the corrective actions and expedite any such actions that
become overdue. Where overdue corrective actions become excessive the matter will be
referred to the Project Manager for further intervention.

5. Where corrective action taken is found to be suitable the reports shall be filed for follow-
up purposes. Where the corrective action requires changes to the safety manual or
procedures, such matters shall be carried out in accordance with document control
procedures.

6. In event of an accident or near miss, the potential causes shall be identified and the
activity shall be resumed only after safety officer’s confirmation of the implementation
of the preventive actions. The accident and near miss reports as necessary shall be
generated.

7. Where potential causes of non-conformities are identified the safety officer shall request
the relevant functions within the project to analyze the potential problem and devise
appropriate preventive action.

8. Proposed preventive actions shall be approved by the Project Manager prior to


implementation.

15.3 HSE PROCEDURE FOR PERIODICAL REPORTING

PURPOSE:
The purpose of this procedure is to detail the methodology for reporting to the employer and to the
legal authorities on matter regarding the health & safety system.

1. In the event of the occurrence of any accident or near miss, the same shall be reported to
the Consultant through the Accident Report and Near Miss Report as appropriate.

2. All documents submitted to the Consultant shall have prior approval from the Project

PROCESS:

General Rules

1. No vehicles shall be allowed to pass or stop near the edges of excavations, steep sloping,
etc.

2. Necessary protection on or adjacent to a street, flagman or other appropriate traffic


control shall be provided.

3. Hand signaling shall be with red flags during the daylight and with red lights during
darkness.

4. The flagman should wear yellow or orange warning garment while flagging. During
darkness the warning garment shall be of a reflecting material.

5. The person authorized to drive a vehicle at construction site should hold a valid driving
licensed for that type of vehicle.

6. Construction areas shall be posted with legible traffic signs at points of hazards.

Personnel Transportation Vehicles

1. All personnel transportation vehicles such as cars shall be parked in the designated car
parking areas, outside the perimeter fencing of the construction area.

2. Personnel transportation vehicles are not permitted to enter the site. They should park at
designated areas only.

3. Appropriate warning signs to caution the drivers while moving out from car parking to
public road shall be installed in car parking areas.

IMPORTANT NOTES FOR VEHICLE DRIVING


Always complete the daily vehicles checks before driving. Check for the oil, tire
pressure, other engine fluids, clean windows, clean headlights and tail lights etc.
Always ensure that your driving license, the vehicles technical passport and the
insurance certificate are in your vehicle.
Always fasten your seatbelt. A seatbelt can save your life and prevent serious
injury. The vehicle shall not move unless all passengers have put their seatbelt.

1. Vehicles for the purpose of transportation of materials intended for construction


activities or for the purpose of disposal of materials from site shall be allowed to enter
the site if practically possible.

2. Necessary caution sign/signals for maximum speed limit 20 Km/h shall be installed at
appropriate locations in the site.

3. Appropriate traffic signals shall be installed at relevant locations so as to guide on the


pass ways & parking areas and caution on the hazards in the construction area.

Construction Equipment

1. The operators of diesel engine driven excavators, moving cranes, etc. intended for use at
site for construction activities, should hold valid permit for that type of vehicle.

2. The maximum allowable speed limit of a vehicle on the construction site may vary
in each emirate. However when not stated, 15 – 20 Km/h may be adapted.
3. Speed limit signboards will be displayed at appropriate locations visible by the
drivers and operators.
4. These vehicles shall be parked only at the designated area for parking on site.

5. Signaling assistance as may be necessary while under construction operation such as


excavation, lifting or during exiting out to the public road shall be provided.

6. The fuelling of these vehicles necessarily to be done at site shall be at the designated
areas free of fire hazards.
7. It shall be ensured that the operators are aware of the following hazards at site:
 Underground cables & overhead lines if any
 Excavations & steeply sloping grounds
 Movement of people
 Other restricted areas

7. Such vehicles should not be used for transporting people or lifting up people.

8. Engine driven light construction equipment with manual pulling, shall be parked at the
designated areas at site. Whenever the equipment is parked, the parking brake shall be
set. Equipment parked on inclines shall have the wheels choked and the parking brake
set. The restricted areas for such equipment shall be indicated.

15.5 HSE PROCEDURE FOR SIGNS, SIGNALS AND BARRICADES

PURPOSE:

The purpose of this procedure is to detail the methodology for use of signs, signals and barricades to
warn of hazards and prevent accidents at the work site.

Signs

Safety Tags

1. Accident prevention tags such as “UNSAFE FOR USE” shall be used as a temporary

Barricades

1. Perimeter fencing / sheeting shall be done for protection of the public from hazards of
the construction site. This fencing shall be capable of withstanding the effects of heavy
winds and similar hazards.

2. Flasher amber lights or floodlights shall be provided as necessary to warn the passer-by
and vehicles of the hazards, especially in darkness.
3. No timber or material with projecting nails shall be used in any work or allowed to
remain in place, as they are a source of danger to persons.

4. Barricades in the form of railing shall be provided for protection of people against the
following:

 Any fall into different levels.


 Openings & manholes where there is a risk of falling down to a distance of more
than 2m.
 Excavations 2m and deeper.
 Material falling from building under construction.
 Building material interfering with pedestrian and vehicular movements in public
areas.
 Any other hazardous area.

5. The standard guard-rail shall be timber or steel frames with the following minimum
dimensions.

 1.15m high
 150mm high toe-board
 Intermediate railing between toe-board and guard-rail at 765mm

6. Suitable strong covers or barriers securely fixed may replace the barricades in case of
openings & manholes and shall be ensured that once removed these are replaced as soon
as practicable.

7. Fencing with red/white colored fencing cones and similar colored warning tapes may be
necessary for the following hazards:

 Excavations less than 2m deep.


 Temporary works carried out near to public traffic.
 Areas having hazards of lesser magnitude as detailed above in barricades.

Signaling

1. When operations are such that signs, signals & barricades do not provide the necessary
protection on or adjacent to a street, flagman or other appropriate traffic control shall be
provided.

2. Hand signaling shall be with red flags during the daylight and with red lights during
darkness. Suitable hand held “STOP” sign with white colored letters on red background
may be used for the same. The flagman should wear yellow or orange warning garment
while flagging. During darkness the warning garment shall be of reflecting material.

3. Persons operating or guiding cranes and hoisting machines should follow the rules for
crane and hoist signal.

PERSONAL PROTECTIVE EQUIPMENT

It is mandatory to wear identification and appropriate personal protective equipment at site as


detailed below:

Head & Foot Protection

1. The entire project work site shall be considered a hard hat & safety shoe area. All
employees & visitors shall be required to wear approved safety helmet and safety shoe at
the construction site.

2. The persons designated for safety by the subcontractors shall be responsible to ensure
that all personnel working at site on their behalf do wear appropriate personal protection
equipment.

3. Visitors to the site should be given the safety helmet and safety shoes on reporting at the
site office. These may be collected back when the visitors leave site.

4. High boots shall be worn by persons engaged in the concreting & shall be ensured that
the trouser covers the boot top to avoid materials falling inside the boot.

Protective Clothing

1. All personnel engaged in the construction work at site including subcontractors should
wear appropriate protective clothing to protect exposed skin area against wounds,
shocks, burns or evaporation.

2. Cotton coveralls with long sleeves shall be used as protective clothing. It shall be
ensured that sleeves are not too loose and the trouser covers the boot top to prevent
materials falling into the boots.

3. Synthetic & oil/chemical soaked clothes shall not be used.

Eye & Face Protection

1. Appropriate Eye & face protective equipment shall be used wherever necessary as
detailed below.

2. Face masks shall be worn by people engaged in operations involving chemicals or other
materials hazardous to health. Some of the operations are as below:

Safety Harness

1. Appropriate safety harness shall be worn by personnel working at heights where there is
a risk to fall beyond 2 meters.

CONTROL

1. The safety officer shall be responsible for ensuring that all personnel at site are equipped
with necessary personal protective equipment and has the authority to withdraw from
site any person violating the rules or suspend any activity involving persons without
adequate personal protective equipment. This shall include the personnel and activities
of the subcontractors also.

2. It shall be the responsibility of the subcontractor’s designated person for safety to ensure
conformance with the procedure.

SCOPE OF APPLICATION

The procedure covers all lifting gears & lifting appliance at the construction site.

DEFINITIONS

Lifting Gear: means a chain, rope sling or similar gear and a ring, link, hook, shackle, swivel or
eyebolt used for lifting the loads.
Lifting appliances: means a grab, winch, pulley block, gin wheel etc. used for raising or lowering
and a hoist, crane, excavator, dragline.

Responsibility for Application

1. The safety officer shall be primarily responsible for adherence to this procedure. The site
engineers, supervisors shall be responsible to ensure that safe material handling methods
are exercised at site.

PROCESS:

CONTROL

1. A list of all lifting appliances and lifting gear shall be prepared and maintained by the
storekeeper. The list shall be updated as necessary.
2. New items shall be added to the list only after inspection and acceptance by the safety
officer.
3. The lifting appliances and gear shall be purchased from approved suppliers only and
shall have a valid test certificate issued by the manufacturer.

The certificate should indicate the following details:

4. The serial number and the safe working load shall be legibly marked on the lifting
appliance & lifting gear.
5. Defective lifting appliances & lifting gear if any at site shall be destroyed immediately to
avoid inadvertent use.
6. All the lifting appliance & lifting gear should be subjected to inspection & test, every 6
months by a third party approved by Ministry of Labour and social Affairs and a
certificate of inspection & test shall be obtained & maintained by the store keeper.
7. The designated person from the subcontractor shall control lifting gear and appliances
used by them & the updated copy of the list shall be forwarded to the Safety Officer of
Amana.

Safe Use of Lifting Gear & Appliance

1. Rigging equipment for material handling shall be inspected by the supervisor or operator
prior to each use and as necessary during its use to ensure that it is safe.
2. Defective rigging equipment shall be immediately removed from service. Any item
painted red as an indication of rejection should not be used for lifting operations.
3. The guidelines for safe material handling are detailed in Annex A
4. The safety officer has the authority to suspend any material handling activity performed
without respect of safety practices.
5. Non-conformance to this procedure shall be handled as per the procedure of control of
non-conforming items.

ANNEX A

GUIDELINES FOR THE SAFE USE OF LIFTING APPLIANCE & LIFTING GEAR

1. Any rigging equipment should be inspected by the respective supervisor prior to putting
into use to confirm that the item is free of defects.

2. No rigging equipment should be loaded above its safe working load nor subjected to
shock loading.

3. The fittings used shall be of a minimum breaking strength equal to that of the sling and
free of all sharp edges that could in any way damage the webbing.

4. All hooks shall be provided with an efficient device to prevent the displacement of the
sling or load from the hook or shall be of such a shape as to reduce as far as possible the
risk of such displacement.

5. The person supervising the lifting operation shall have adequate knowledge of the
following:

 Estimation of weight of the load if necessary


 Estimation of Load Centre of gravity & load stability
 Determination of safe working load of slings in different configuration of slinging &
the angle of slinging.

6. Slings should not be shortened with knots or bolts or other makeshift devices. Wire
ropes should not be secured by knots.

7. Care shall be taken to securely attach the slings to the loads and lifting appliance &
suitable packing shall be used to prevent the edges of the load from coming into contact
with the sling, rope or chain where this would cause danger.

8. Hands & fingers shall not be placed between the sling and its load while the sling is
being tightened around the load

9. A sling should not be pulled from under the load when the load is resting on the sling.

10. Nylon web slings shall not be used where fumes, vapors, sprays, mists or liquid of acids
are present.

11. Polyester and polypropylene web slings shall not be used where fumes, vapors, sprays,
mists or liquids of caustics are present.

12. When not in use, all handling gear and appliances shall be removed from the work area
and stored properly at designated locations, free from hazardous materials and weather
conditions.

13. Poles and beams used for the suspension of chain blocks or gin wheels shall be of
adequate strength and the chain block or gin wheel shall be effectively secured to the
pole or beam. The poles and beams shall be properly secured against movement.

14. Where a hoist is operated by means of a winch, the winch shall be so constructed that the
brake is applied when the control lever handle or switch is not held in the operating
position.

15.8 HSE PROCEDURE FOR SAFE SITE ELECTRICAL INSTALLATIONS

PURPOSE:

The purpose of this procedure is to define the methodology for safe electrical installations at site for
construction.

RESPONSIBILITY OF APPLICATION

The site engineer shall be responsible to establish the site electrical installations conform to this
procedure.

The safety officer shall be responsible for periodical inspection. The safety officer has the authority
to physically remove any installation / equipment or cable termination, which can cause a safety
hazard.

PROCESS:

Design & selection of electrical distribution system

1. The design and selection of the electrical distribution system for construction site shall
be done by a competent electrical engineer.

2. All electrical apparatus and accessories used at site shall be in accordance with ADWEA
regulations for electrical installations.

The electrical distribution system consists of the following:

3. Necessary protective devices against overload, short circuit and earth leakage shall be
incorporated in the system.

4. All distribution panels shall have lockable doors to prevent unauthorized operation or
terminations and isolators should be lockable types at OFF position. All equipment shall
be adequately protected against direct or indirect contacts.

5. Cable terminations shall be with proper size crimping sockets / lugs wherever necessary.

6. It is prohibited to lay cables directly on ground, causing a safety hazard.

7. Long supply cables for movable plants & portable tools should be protected against
mechanical damages. Such cables shall be either installed in metal conduits / pipes or
supported on posts.
Earthing

1. Copper earth electrodes of 20mm or 16mm diameter, 3 m deep shall be installed in earth
pits and connected to the main distribution board.

2. The earth resistance value should not exceed 1 ohm.

3. The earthing conductor of the incoming supply or that of the generator shall be
connected to the grounding system.

4. The earth continuity conductors shall be installed to all sub, final distribution boards &
all fixed electrical equipment.

5. Supply cables to movable and portable electrical equipment / tools and extension cords
shall have an earth core and should be effectively connected to the earth terminal in the
source panel.

Inspection & Testing

1. Earth resistance of the main earth electrode shall be measured upon installation and
frequently every 3 months.

2. The earth leakage protective devices shall be tested with the test button every month and
before energizing any new installed circuit.

3. Earth leakage protective devices shall be tested using the earth leakage tester every six
months and at the initial installation stage prior to energizing.

The following inspections & tests shall be carried out on each circuit before energizing:

 Proper installation & termination


 Protection against direct and indirect contact with active parts.

 Insulation resistance measurement.
 Earthing continuity
 Tests on earth leakage protection devices if any

The results of all inspection & tests shall be recorded.

Diesel Generator Units

1. Prior approval from ADWEA shall be obtained for the use of diesel generator units at
site.

2. The neutral of generator units used for the purpose of generation of electricity, shall be
effectively grounded.

3. A competent person shall be designated for the operation and maintenance of the diesel
generator equipment.

4. Fuel storage for the units on site, if any, shall be protected against fire hazards.

5. Adequate means of protection, isolation & changeover shall be provided in case the generator supply is
connected to the site electric distribution system. An instruction board shall be posted for such control
operations near to the appropriate switchboard.

15.9 HSE PROCEDURE FOR CONTROL OF CRANES AND FORKLIFTS

RESPONSIBILITY FOR APPLICATION

The safety officer shall be primarily responsibility for adherence to this procedure. The site
engineers, supervisors shall be responsible for ensuring that safe material handling methods are
exercised at site.

Own Equipment

1. All the heavy equipment under the scope of this procedure owned by Amana Steel
Buildings Contracting Co. L.L.C. are maintained and controlled by the maintenance
department.

2. It shall be the responsibility of the head of maintenance to ensure that all such equipment
are subjected to periodical inspection & tests by an approved competent person and hold
a valid certificate thereto.

3. The Safety officer at site shall ensure that the equipment in use at site have a valid
inspection & test certificate and the operators hold valid permit issued by the traffic
authorities for that type of equipment.

1. It shall be the responsibility of the safety officer to ensure that all equipment rented by
the company for use at site hold a valid inspection and test certificate issued by a
competent person and the operator hold a valid permit issued by the traffic authorities
for that type of equipment.

2. The safety officer shall maintain copy of the certificate

SAFETY AT INSTALLATION AND USE

Mobile cranes with site assembled lattice boom shall be installed and commissioned by a third party
governed by legal requirements. All inspections & tests necessary shall be conducted and a
certificate with effect shall be obtained.

The safety officer has the authority to suspend any activity at site engaging unsafe heavy equipment
or operators exercising unsafe practices.

All equipment shall be operated in accordance with manufacturer’s operating manual, which shall
be retained in the cab.

Rubber tire, self-propelled material handling & construction equipment at site shall be equipped
with rollover protective structures to minimize the possibility of the operator being crushed as a
result of a rollover or upset.

All bi-directional equipment shall be provided with a horn, distinguishable from the surrounding
noise level.

The forklift trucks shall be provided with an automatic reverse activated horn or buzzer and light.

The guidelines on safe working methods for cranes at site are detailed in Annex A.

Any non-conformance to this procedure shall be handled as per the procedure of handling non-
conforming items.

ANNEX A

GUIDELINES FOR THE SAFE USE OF HEAVY EQUIPMENT AT SITE

2. Cranes shall not be used or erected under weather conditions likely to endanger their
stability such as heavy wind, etc.

3. The hoisting mechanism of a crane shall not be used for any purpose other than raising or
lowering a load vertically.

4. Crane jibs shall not be worked at a radius greater than specified on the test certificate.

5. Mobile cranes without riggers / signalman should not lift or slew loads. Mobile cranes
should be adequately positioned on a suitable ground with level bearing pads.

6. Mobile crane hydraulic booms must be fully retracted and not raised more than 30 degree
before moving free on rubber, unless dictated by operation requirements.

7. No person shall be allowed to work or stands near the operating boom or the tail swing of an
excavator.

8. When the excavator is being adopted for use as a crane, slings shall be secured to a correctly
designed and manufactured lifting point and it shall be ensured that no person shall stand
under the load during lifting.

9. Care shall be taken to ensure the stability of the equipment while in operations or at parking.

15.10 HSE PROCEDURE FOR SITE LIGTHING

PURPOSE:

The purpose of this procedure is to detail the methodology for site lighting.

SCOPE OF APPLICATION

The procedure covers the complete lighting requirements at the construction site including offices.

1. Adequate lighting shall be provided at all areas of the construction site sufficient for safe
movement of men and vehicles.

2. Every part of the area shall receive light from at least two directions to avoid dangerous
dense shadows.

3. Construction areas, ramps, runways, corridors, offices, shops and storage areas shall be
lighted to not less than the minimum illumination levels listed below:

vehicles inside the site.

6. Areas under excavation, demolition and workplaces at site shall be sufficiently


illuminated for safe working during darkness.

7. Workshops, steel fabrication, carpentry workshops and similar work areas should have
sufficient lighting. Offices, electrical control rooms & eating places shall be adequately
lighted.

8. Outdoor material storage, off loading areas & storage areas for waste materials should
have sufficient lighting.

9. Adequate lighting shall be provided at the outdoor electrical switch, generator control &
fuelling areas.

The following points shall be taken care of:

 Workplaces and walkways should be lit to avoid glare and deep shadows
 Fluorescent lamps shall be fitted with their long axial in line with walkways

 Lighting should be located higher than the work and as possible inaccessible other than
to the electrical technicians
 All light fittings and the support poles shall be effectively earthed except CL2 and CL3
type.

10. The general outdoor lights shall be pole mounted weatherproof type HPMV lamps or
tungsten halogen lamps.

11. Fluorescent lamps shall be used for all indoor lighting applications and where the
luminaries are installed at a lower level.

12. Emergency lights shall be provided in walkways, escape routes and other emergency
locations.

15.11 HSE PROCEDURE FOR CONTROL OF LADDERS

PURPOSE:

The purpose of this procedure is to ensure safe control and use of ladders at site.

The procedure covers the control and use of ladders at the site including subcontractors.

 Ladders used at construction sites shall be designed for extra heavy industrial use..
 All hinged ladders shall have four supporting rails or legs.
 Folding ladders that have a vertical extension ladder at the top step of the ladder are
prohibited.
 Ladders should have firm footing and provision for firm supporting.
 Single ladders must not be longer than 6m.
 Extension ladders must not be longer than 11m.
 Step ladders and platform ladders must not be longer than 3.5m.
 All single section and extension ladders must have non-slip hinged pads or
assemblies on the base of the side rails / strings.
 The top platform of the ladder shall be marked to indicate it is not a step.
 Ladders with missing or broken rugs, broken or split side rails, missing non-slip feet
or otherwise damaged shall not be used.
 Bends, dents, cracks, loose or missing rivets, disconnected braces and corrosion
weaken a ladder seriously.
 The area around rivet points on fiberglass ladders should be checked for hairline
stress cracks.
 Wooden ladders with projected nails or sharp, dangerous edges should not be put
into service.
 Wooden ladders may not be painted except for the platform and top step.
 Wooden ladders should be protected with a preservative seal whenever possible.

Permanently Fixed Access Ladders

Ladders installed for access to different levels shall be subjected to inspection by the safety officer
prior to use.

In addition to the checkpoints the following shall be verified:

 The ladders shall be capable of carrying the desired load without any damage or
displacement,
 The top of ladders shall extend one meter above landing surfaces or platforms when
this is not practical, grab rails shall be installed.
 The ladders shall be tied, blocked, or otherwise secured to prevent accidental
displacement, at top & bottom.
 The access areas at the top and bottom of ladders shall be kept clear of obstructions.
 Ladders shall not be used in the horizontal position for a walkway or work platform.
 Unsafe ladders shall be immediately removed from site.

Safe Use of Ladders

Ladders shall be inspected by supervisors or the users prior to use.

 Missing or broken rugs, broken or split side rails / supports, missing non-slip feet or
otherwise damaged shall not be used.
 Wooden ladders shall be checked for presence of unsafe projections of nails or sharp
edges.
 Metallic ladders shall not be used when personnel are working near or on electrical
systems.
 Personnel shall not stand on the top of stepladder.
 Personnel shall not carry materials and tools in hands while climbing or descending
ladders.
 Two or more people shall not work from the same ladder.
 Ladders should not be kept in areas of door opening.
 Extension and straight ladders shall be installed so that they have 1m of base for
every 4m of height.
 Ladders must be stored where they will be protected from damage, usually hung on
brackets in a horizontal position.
 Care shall be taken while carrying ladders not to hit on personnel and structures.

15.12 HSE PROCEDURE FOR CONTROL OF SCAFFOLDING

PURPOSE:

The purpose of this procedure is to ensure safe control and use of scaffolding at site.

SCOPE OF APPLICATION

The procedure covers the control and use of scaffoldings at the site including subcontractors for
height of 2 meters and above.

RESPONSIBILITY FOR APPLICATION

The HSE Officer and Scaffolder Inspector shall be responsible for ensuring that only safe
scaffolding are in use and are in used in accordance with this procedure.

Provision of scaffolds and means of access

1. Where work cannot safely be done on or from the ground or from part of a building or
other permanent structure, appropriate and suitable scaffoldings shall be provided for
heights 2 meters and above.

2. Areas where work require less than 2 m height, stable tables/benches with proper
bracing and adequate width and length shall be used after safety officer’s inspection and
approval.

3. The design of the scaffolding system shall be done depending on the requirement at site,
taking into account all the safety aspects including type of work, load height and weather
conditions.

4. The scaffold and every part thereof shall conform to the international standards.

Control

1. Scaffolding and components shall be subjected to inspection by the safety officer or the
designated competent person, prior to use.

2. Metal parts used for scaffolds shall be of suitable quality and free from corrosion or
other patent defect likely to affect their strength.

3. Missing, defective, broken, corroded, bend, worn out or otherwise damaged scaffolds or
components shall not be used at site.

4. Appropriate visible identification shall be put on the items found to be safe for use.

5. Defective or unsafe items shall be segregated, suitably identified with a “NOT SAFE
FOR USE” tag to prevent inadvertent use and shall be removed from site.

6. The storekeeper shall be responsible to ensure that only inspected and approved
scaffolds are issued to the site for use.

1. Scaffolds shall be erected or substantially added to or altered or dismantled under the


immediate supervision of designated competent workmen.

2. It shall be ensured that only components approved by the safety officer or the designated
person as safe to use be used for assembly of scaffoldings.

3. Partly erected or dismantled scaffoldings shall be suitably identified with a warning to


prevent inadvertent use.

4. Scaffolds shall be properly maintained. They should be regularly inspected by the safety
officer. Unsafe scaffoldings shall be identified and repaired.
Roller Scaffoldings

1. Scaffolds, which can be moved on wheels or skids, shall be used only on firm and even
surface not so sloping as to involve risk of instability of the scaffold or any load thereon.

2. These shall be secured to prevent movement when any person is working on it.

3. These shall be moved only by the application of forces at or near their base.

Working Platforms

1. Adequate work platforms with toe boards shall be installed for persons doing work at
height and / or deposit of materials.
2. Every side of the working platform or working place from where a person is liable to fall
to a distance of more than 2m shall be provided with guard rails or otherwise a secure
handhold shall be provided along the full length of the platform.

3. The working platforms and guardrails shall conform to the requirements listed below:

The purpose of this procedure is to ensure safe use of electrically operated tools and equipment on
site.

SCOPE FOR APPLICATION

The procedure covers electrical operated hand held tools and permanently installed equipment at
site including that of the subcontractors.

RESPONSIBILITY FOR THE APPLICATION

1. The safety officer shall have the primary responsibility to ensure that only safe tools &
equipment are in use and are used in accordance with the provisions of this procedure.

2. The site engineers, supervisors and persons responsible from the subcontractors shall
ensure that these tools and equipment are operated safely.

PROCESS:

CONTROL

1. A list of electrically operated hand held tools and permanently fixed equipment at site
shall be maintained by the storekeeper. Periodical updating of the list shall be done on
addition or deletion.

2. The list of such tools and equipment to be used at site by the subcontractors shall be
obtained and maintained by the safety officer.

Inspection

1. Prior to issue for use at site all electrical tools and equipment shall be inspected and
tested as per Annex A: Checklist for inspection and test of electrical tools & equipment.

2. Defects if any, which competent person at site can correct and the item shall be inspected
prior to re-use.

3. Tools & equipment found safe for use should be suitably identified and stored at a
designated location free from factors causing damage.

4. Unsafe items shall be segregated and removed from site.

5. The checklist for inspection & test for all the tools and equipment in use at site by the
subcontractors shall be obtained and maintained by the safety officer.

Safe Use of Electrical Tools & Equipment

1. It shall be ensured that any electrically operated hand held tool or fixed equipment is
operated by personnel competent to operate the same. The operators shall be aware of the
functions, controls, emergency devices and the possible hazards with the equipment.

2. The operators shall use appropriate personal protective equipment while using the tool.

3. It shall be ensured that electrical safety devices such as overload, short circuit or earth
leakage protections and mechanical protections like guards for moving parts are not
removed from the equipment.

4. Lengthy cables for movable plant and portable tools shall be protected from mechanical
damage.

5. Supply cables for fixed plants shall be routed clear of construction work and be protected
against traffic movement.

6. Use of unsafe cables & cable joints, broken / damaged connectors, plug & sockets or any
unsafe accessory shall be strictly prohibited.

7. Connection without or with incorrect plug sockets, group connection of equipment in one
socket outlet, damaged or spliced cord / extension etc. are prohibited.

8. No circuit should be overloaded. Precaution shall be taken against loose connection of


cables.

9. Industrial weatherproof connectors, socket outlets and plug connectors shall be used.

10. The tools shall be disconnected from supply before any adjustment is made or attachments
changed.

11. The operator should put the tools off / on and remove connector from socket outlet upon
completion of work.

Periodical inspection and maintenance

1. The storekeeper is responsible for maintaining the electrical powered tools in good
condition.

2. For permanently installed electrical equipment the responsibility for maintaining in good
condition rests with the supervisor of the concerned section.

3. All the electrical tools and equipment shall be subjected to monthly inspections as per
the checklist for inspection & test of electrical tools and equipment.

15.14 HSE PROCEDURE FOR FIRE PREVENTION

PURPOSE:

The purpose of this procedure is to detail the methodology for ensuring fire safety at the
construction site.

SCOPE OF APPLICATION

The procedure details the fire protection systems at construction sites and the fire protection
programmed to be followed throughout all phases of the construction work.

RESPONSIBILITY FOR APPLICATION

The safety officer shall have the primary responsibility to ensure compliance with the provisions of
this procedure.

All personnel working at site shall be responsible to adhere to this procedure.

1. The complete construction area shall be installed with appropriate perimeter fencing and
security shall be provided at entrances to prevent the unauthorized invasion of public.

2. Internal combustion engines powered equipment shall be so located that the exhausts are
well away from combustible materials.

3. Fire checks shall be undertaken throughout during holidays and weekends.

4. Written emergency procedures should be displayed in prominent locations and the


information they contain should be highlighted to all employees on the site.

Carelessness in smoking or with lighted matches

1. Smoking is strictly prohibited at the entire construction site. Carrying cigarette, lighter or
matchboxes to the site is not permitted.
Cooking fires from canteen and kitchen

1. Canteens with cooking fires are not allowed on site.

2. In kitchens associated with the site offices, adequate measures shall be taken to ensure
safety against fire.

Hot work

1. Prior to start of any hot work, including welding, gas / arc cutting, heating etc. the
following shall be ensured:

 The safety officer is advised and aware of the work


 The area is sufficiently far from buildings/structures or material storage areas
having a fire hazard
 Appropriate fire extinguishing devices are made available near the work area
 Adequate escape routes are identified and are free of obstructions
 All personnel engaged in work wear appropriate personal protective equipment
 All electrical equipment and accessories are safe and all safety devices are in
service
 All electrical connections are adequate to avoid loose connection and sparking.

2. Fire check shall be conducted by the supervisor/competent person on completion of all


hot works.

3. Temporary buildings, structures shall be to the extent possible of non-combustible


construction. Otherwise, steps shall be taken not to keep combustible materials in such
buildings.

Electrical Faults

1. The site electrical installations shall conform to the procedure for safe site electrical
installations.

2. All electrical connections shall be free of loose connection or causing short circuit. HRC
fuses should not be replaced with wire.

3. It shall be ensured that all electrical works at site are done by competent personnel only.

4. It shall be also ensured that all electrical equipment used is in accordance with the procedure
for control of electrical tools & equipment.

Faulty or misused heating equipment

1. It shall be the responsibility of the site Engineer, to ensure that all heating equipment
used at site are safe to use and are used by competent personnel only.

2. The storekeeper shall be responsible for ensuring timely disposal of all accumulated
wastes / rubbish from the site.

Material Storage

1. Locations for storage of materials in open area shall be properly designated.

2. Materials shall be stored to minimize the spread of fire internally and to permit
convenient access for fire fighting. Aisle space shall be maintained at all times to safely
accommodate the widest vehicle that may be used within the building for fire fighting
purpose.

3. No combustible material shall be stored in temporary buildings.

4. Combustible materials shall be stacked with due regard to the stability. Combustible
materials shall be stored outdoors 3m away from a building or structure.

5. Portable fire extinguishers for the type of fire hazard involved shall be provided at

2. Containers and portable tanks made from suitable material for storage and handling of
flammable and combustible liquids shall be used. These shall be labeled.

3. Not more than 60 gallons of flammable or 120 gallons of combustible liquids shall be

Type Method of Use
Water Direct the jet at the base of the flame and keep it moving across the area of
fire. Seek for any hot spots after main fire is extinguished.
Dry Powder Direct the discharge towards the near edge of the fire and with a rapid
sweeping motion, drive the fire towards the far edge until all flames are
extinguished.
On fires in falling liquids, direct the discharge at the base of the flames and

2. The presence of warning signs for underground services such as warning tapes etc. shall
be thoroughly observed.

3. The excavations when done with powered equipment (diesel engine driven shovel, etc.)
shall be under the supervision of the supervisor / foreman or any other person authorized
by the site engineer, to avoid accidental damage to underground services

4. All emergency measures shall be taken to ensure safety in case of damage of any of the
underground services during excavations

5. The walls and faces of all excavations in which employees are exposed to danger from a
fall or dislodgement of earth materials shall be guarded by appropriate shoring systems.
All struts and braces shall be properly and adequately secured. Bracing or shoring of
trenches shall be carried along with the excavation. Portable trench boxes of sliding
trench shields may be used for the protection of personnel in lieu of a shoring system or
sloping.

6. In case of possible instability of adjacent structures, adequate steps such as shoring,


bracing etc shall be taken.

Personal Protection Equipment

1. All personnel working at excavations shall have the following personal protection
equipment:

2. When employees are required to be in trenches more than 1.2m deep, adequate means of

Fencing

Identification of Hazardous Substances

1. Hazardous substances are identified by the manufacturer / supplier with a warning label
on the container / package nor harmful, corrosive, toxic, irritant, flammable or any
combination of these.

2. The chemical container shall have the name of chemical, symbols & words identifying
the chemical.

3. While applying the chemical, a signboard will be displayed and all workers will be kept
away from the area of application.

Control of hazardous substances

1. No harmful chemical shall be used unless an alternative is not possible.

2. On receipt of any item as identified by the manufacturer as hazardous as explained


above, the same shall be reported to the safety officer.

3. The safety officer or a competent person authorized shall obtain the following details

hazard in an open flame activities.
Proper PPE shall be provided for the workers for all the types of welding and cutting.
Suitable Fire Extinguishing Equipment shall be immediately available in the qork area
and shall be maintained in a state of readiness for instant use.

Oxyacetylene Welding Procedure

Blow out cylinder valve before connecting regulator, pressure regulator including
their gauge should be in proper working order while in use.
Release adjusting screw before opening the valve.
Stand on one side of the regulator. Flash back arrestor shall be fitted on the regulator.
Open the cylinder very slowly.
Do not use acetylene in a free state at a pressure above 15 psi.
Purge acetylene and oxygen passages individually before lighting. Light acetylene
then opens the oxygen on torch. Clogged torch tip openings shall be cleaned with
suitable cleaning wires, drills or other devices designed for such purpose.
Torches shall be lighted by friction lighters.
Never use oil/grease or oily hand gloves on regulator, hose and apparatus and on tips
that contact oxygen.
Do not use oxygen as a substitute for air.
Keep combustible out of your work area.

Site Illuminations

 At least 50 lux minimum illumination shall be provided and maintained at night shift
activities in order to prevent potential risks that might occur particularly.
 Every part of the area receive light from flood light at least two direction to avoid
dangerous dense shadows.
 Adequate lighting shall be provided to enable operation to proceed safely during
darkness.

Plant & Materials

 All construction equipment shall be equipped with reverse horn or a banksman shall be
assigned to guide the reverse movement.

Site Emergency Procedure

 The purpose of this procedure is to ensure that all accidents and incidents are reported in
a logical and consequent manner to give guidelines on the preparation of preliminary
accidents reports and investigation reports.
 In the event of an accident the following action shall be taken by Amana Contracting &
Steel Building
Refer to the HSE Emergency Procedure attached on the Project Safety Plan
 Emergency contact numbers shall be displayed on prominent locations on site and site
offices.

 ACSBC shall arrange for a suitable transportation facility for transporting casualties to
the nearest government hospital as required. ACSBC shall provide the project work site
with all necessary first aid clinics, equipments and medicines. First Aid clinics shall be
in attendance of project Nurse and First Aider Trained by Third Party at all times when
the work on site is in progress.

Fire Prevention

 Fire extinguisher shall be provided at strategic locations.


 The site operates a “No Smoking” Policy” corresponding disciplinary action shall be
given against the violator.
 Strategic locations of fuel tank and diesel drums shall be identified.

15.19 MEDICAL EVACUATION PROCEDURE

Points to remember for Medical Evacuation Procedure

1) If safe to do so the Charge hand must remove/eliminate the danger;


2) Charge hand must apply first if qualified;
3) Charge hand must nominate a worker to inform the First Aid Officer;
4) The First Aid Officer must assess victim and determined whether to attend the victim in the
first aid office(if mobile) or where they lay ( if immobile);
5) Whilst First Aid is being applied by the First Aid Officer, the charge hand must inform the
Project Manager and Project Management (Parsons) of the injury;
6) Project Manager shall call ambulance to pick up the victim from site;
7) Charge hand shall nominate a worker to wait at the main entry gate to the ATI and
Schlumberger Site (gate with permanent guardhouse) to direct the ambulance to the site;

8) Charge hand shall wait at the site entry for the ambulance and once on site to direct to the
victim;
9) First Aid Officer to treat victim until ambulance arrives;
10) First Aid Officer shall be present in the ambulance and at the hospital to ensure that the
victim gets swifts treatment.

OR

Should an ambulance not be available within a reasonable time, the following procedure
must be followed:
10.1 Project Manager shall liaise with Mafraq Hospital (Primary Hospital) or a secondary
hospital to ensure that the victim can be immediately treated at that hospital;
10.2 Driver shall take the victim to the designated hospital in Amana’s first aid van;
10.3 First Aid Officer must escort the victim in the van and to continue first aid treatment;
and
10.4 First Aid Officer must assist the victim at the hospital to ensure that the victim gets
swift treatment.
11) The relevant authorities shall be informed as to the accident;
12) Amana shall prepare a report listing details of the accident that shall be submitted to
Schlumberger and Parsons Limited;
13) Schlumberger and Parsons Limited may make suggestions to prevent the accident from
happening again;
14) Amana rectify any causes of the accident; and
15) Amana to educate their employees to ensure that the type of accident does not happen again.
