SUBJECT : MONETIZATION

Greetings!

1. This is to request for monetization of my leave credits of sixty (60) days;

2. That the proceeds of the above request would be used for pre-BAR
expenses (inclusive of review center payment and application
requirements and fees), plane fare expenses, accommodation, daily
allowances and other miscellaneous expenses of my spouse during her
BAR examinations scheduled on October 7, 14, 21, and 28, 2012 in
Manila;
3. That at present, I had accumulated about 71.166 leave credits, the same
is subject to verification from the HRIAD;
4. Hoping for your favorable approval of the above request.
