Important Reminder:
1. The student must submit the accomplished request form and pay the amount of Php 200.00 at the
SAM office on or before April 5, 2019 (Friday).
2. All students with academic deficiencies are required to bring their parent or guardian to the SAM
office before they can be given a schedule of enrollment.
3. Status of accomplished request forms and schedule of enrollment will be sent through email.
4. For inquiries or clarifications, you may call Ms. Santos at the SAM office at 735-9443 from
Monday to Friday, 9-11 am.
