AFFIDAVIT OF CONSENT

I, ADONES ROSTICO R. MADRIAGA of legal age, married, Filipino

Citizen and all resident of Brgy. 25 Callaguip, Paoay, Ilocos Norte, Philippines,
after having duly sworn to in accordance with law do hereby depose and states
that:

1. That I am the declared owner of a certain parcel of lot

designated as Lot No. 7806 situated at Brgy. 25 Callaguip,
Paoay, Ilocos Norte, containing an area of 1,470 square meters,
more or less – covered by Tax Declaration No. 14-021-0053
with a Property Identification No. 003-15-021-02-013-
0000;

2. Therefore, I am the owner of the above-mentioned property;

3. That I am executing this affidavit in order to give my consent to

my son ADONIS RUSTICO R. MADRIAGA JR. to erect a
residential building on the above-mentioned property;

4. That I am execute this Affidavit to attest and declare the

truth of the foregoing facts and for whatever legal purposes
that this Affidavit may serve.
