Dear Mr. Ortega,

I am writing for and in behalf of my client Chief Engineer Winny R. Bedonia who is
currently on board pursuant to a contract with your crewing company.

My client was charged of a case docketed as POEA Case No. DAW 19-04-1071 filed by
RUPERTO F. HERMANO for violation of Section 128B (16-Grave Abuse of Authority)
and (26-Violation of the Code of Discipline for Seafarers) and he availed of my legal
services to file an answer and dismiss the disqualification order issued by POEA.

In relation to this, we respectfully request from CARDIFF CREWING FILIPINAS, INC.

through you, documents, certification and other relevant records to support and help
establish a defense against the case filed. Specifically, we request the following
documents:

1. Official Investigation Findings pertaining to the afore-mentioned case either

reflected through the message exchanges initiated by your crewing manager,
Ms. Ma. Rayie E. Sanvictores or through an official investigation report;
2. Original copy of the POEA Complaint received by Cardiff for and in behalf of Mr.
Bedonia;
3. Voluntary Statement and Incident Report relevant to Mr. Hermano’s jump ship
incident;
4. Certificate of Service for Mr. Bedonia; and
5. Other records or supporting documents relevant to the case.

Your cooperation will greatly help exonerate Mr. Bedonia from the unfair and
whimsical case filed against him. Thank you so much in anticipation of a positive and
