A. Product Specification
Globe’s myBusiness Internet – FIBER DSL delivers Internet connectivity
through wired transmission.

B. Terms and Conditions

1. Pricing (Vat Inclusive)

● Includes a FREE Landline with unlimited calling to Globe/TM Subscribers for the 1st 24
months.

2. Proposal Validity
This proposal shall be valid within 30 days from date of receipt.

3. Contract Term and Pre-termination fees

The contract term for the proposed Service shall be for 24 months. Contract term shall be
automatically renewed for the same period unless the Customer sends a written notice
terminating the Service at least sixty (30) days prior to expiration of the initial contract term,
or renewed contract term, as the case may be. In case of any cancellation prior to the
expiration of the contract term, Customer shall be charged Php2,500 + (2months MSF)

C. Customer Responsibilities
In order for the work order to be encoded, the Customer shall submit the following
documentary requirements upon acceptance of this proposal:
● Globe myBusiness Application Form
● Proof of Company Registration: Business Permit or SEC Registration or BIR Registration
Form 1602 or Latest Audited Financial Statement or electric Bill
● The Customer shall ensure that the following are available at the time of installation:
- Local Area Network equipment, as needed and inside wiring
- Commercial power (AC) and UPS for the modem

D. Confidentiality of Information
The information presented herein shall not be duplicated, used or disclosed in whole or in part
for any purpose without the prior written consent of Globe Telecoms. The Customer shall have
the right to duplicate and use the information only for its internal evaluation of this proposal,
and all information shall not be disclosed, in any manner or form, outside of The Customer.
