AFFIDAVIT OF PATERNITY AND USE OF FATHER’S

SURNAME

I, ERNITO ALEJANDRO MIRASOL, of legal age, Filipino citizen, and presently

residing at St. Mary St. Signal, Taguig, after having been duly sworn to in accordance
with law, do hereby depose and say:

1. That I am the biological father of ERJAN KURT VALDEZ MIRASOL;

2. That my son ERJAN KURT VALDEZ MIRASOL was born on February 20,
2020 at Signal, Taguig City;

3. That being the father of ERJAN KURT VALDEZ MIRASOL, I hereby

acknowledge and recognize ERJAN KURT VALDEZ MIRASOL as my son
and legal heir;

4. That I am allowing my son ERJAN KURT VALDEZ MIRASOL to use my

surname to be his surname in his Certificate of Live Birth; and

5. That I execute this affidavit to attest to the truth of the above facts in compliance
with the provisions of Republic Act No. 9255.
