AFFIDAVIT OF DESISTANCE

We, SPOUSE JUANITO P. TAGNAWA AND AUREA F. TAGNAWA AND REA

JOY T. BAJADOR Filipino Citizens,of legal ages, and resident of Prk. EMB Homes
Labangal General Santos City, after having been duly sworn to depose and say that;

1. We are the driver,passengers and the mother of minor MINE CHARLES

DAVID T. BAJADOR and guardian of minors AARON JAMES TAGNAWA
and FRANZ UANNE JAE TAGNAWA of Haojuo 125 tricycle bearing Plate
No.3119 LX in vehicular accident last February 24, 2018 around 6:25 in
the morning along Maharlika NHW in front of Princess Haina Store,Prk.
Palen, Brgy. Labangal, General Santos City, the Toyota Hilux bearing the
Plate No. MEX 789 driven by ROFER JOHN CASTILLO BELIMAC hit our
tricycle and incurred injuries and damages;

2. IVY JOY MONTEMAYOR has paid the damages amounting of SEVEN

HUNDRED THOUSAND PESOS (700,000.00) incurred in relation to said
accident and we have already amicably settled the disagreement between us;

3. By reason thereof, We will not further filing any complaint against IVY JOY
MONTEMAYOR and the ROFER JOHN CASTILLO BELIMAC in relation to
the accident;

4. We are executing this affidavit FREELY and VOLUNTARILY to attest to the

truth of the foregoing facts and for the purpose of apprising the authorities
concerned, and for whatever legal purpose this may serve.

Land, Lagao, General Santos City, after having been duly sworn to depose and say
that;

5. I am the driver the of motor vehicle owned by SALMA A. SUMNDAD bearing

with Plate no.V2 0341;

6. JOHAN LOUIS H. KOORING has paid all the damages incurred in relation
to said accident and we have already amicably settled the disagreement
between us;

7. By reason thereof, I will not further filing any complaint against JOHAN
LOUIS H. KOORING in relation to the accident on June 15, 2017 at about
10:00 in the morning along the national highway fronting MCDonald,
General Santos City;

8. I am executing this affidavit FREELY and VOLUNTARILY to attest to the

truth of the foregoing facts and for the purpose of apprising the authorities
concerned, and for whatever legal purpose this may serve.
