FOR DECEMBER REGISTRATION

**Please bring a copy of the child’s proof of age and proof of Florida residency to be left
with the application completed at the registration site.
The birth date range for participating in the 2010 Summer Program is September 2, 2004 – September 1, 2005.
The birth date range for participation in the 2010-2011 School Year Program is September 2, 2005 – September 1, 2006.

Directions:
From the Turnpike: Exit west at Commercial Blvd. and continue to NW 64th Avenue (NW 64th Avenue turns into SW 81st
Avenue). Turn right (north) on NW 64th Avenue and continue through the next four traffic lights. Family Central’s office is a four
story white office building on the right (east side) just past the intersection of Kimberly Blvd. and 81st Avenue.

From I-95: Exit west at Cypress Creek Rd. (Cypress Creek Rd. turns into McNab Road) and continue to SW 81 st Avenue. Turn
right (north) on SW 81st Avenue and continue north through the next traffic light. Family Central’s office is a four story white office
building on the right (east side) just past the intersection of Kimberly Blvd. and SW 81st Avenue.
