NEW APPLICATION: BASIC PASSPORT REQUIREMENTS FOR FIRST-TIMERS (ADULTS)

1. Confirmed DFA appointment. Set a DFA appointment here. (Read: How to Pay for DFA Appointment)
2. Print out of accomplished application form. See the Philippine passport Application form online
3. Two (2) Valid IDs with one (1) photocopy each. You may bring any of the following:
1. School ID – current school year
2. Senior Citizen ID
3. SSS (Social Security System), GSIS (Government Service Insurance System), or UMID (Unified
Multi-Purpose Identification) card
4. Driver’s License. If in card format, a Student Permit may also be accepted.
5. PRC (Professional Regulatory Commission) ID
6. OWWA or Overseas Workers Welfare Administration or iDOLE (Integrated Department of Labor
and Employment) card
7. Voter’s ID or Voter’s Certification with dry seal
8. PNP (Philippine National Police) Firearms License
9. Airman License – if issued August 2016 onwards
4. Original copy of PSA (Philippine Statistics Authority) Authenticated Birth Certificate (Read: How to Apply
for PSA Birth Certificate)
Additional documents:
1. For married women taking their spouse’s last name – PSA Authenticated Marriage Contract or Report of
Marriage
2. For Muslim applicants – Certificate of Tribal Affiliation from NCMF (National Commission on Muslim
Filipinos)

PASSPORT RENEWAL: BASIC REQUIREMENTS (ADULTS)


1. Confirmed DFA appointment. Set a DFA appointment here (Read: How to Pay for DFA Appointment)
2. Print out of accomplished application form
3. Current passport with one (1) photocopy of the data page
4. Original PSA documents
1. Annotated Birth Certificate
2. Marriage Contract
3. Annotated Marriage Contract (name change due to dissolved marriage)
4. Spouse’s Death Certificate
5. Any of the following valid IDs with one (1) photocopy
1. School ID – current school year
2. Senior Citizen ID
3. SSS (Social Security System), GSIS (Government Service Insurance System), or UMID (Unified
Multi-Purpose Identification) card
4. Driver’s License. If in card format, a Student Permit may also be accepted.
5. PRC (Professional Regulatory Commission) ID

If your passport is an electronic passport (ePassport), you don’t need to bring an ID and your PSA birth
certificate. You can tell if your passport is an ePassport if the cover is dark maroon in color and it has the
ePassport logo.

CONFIRMING APPOINTMENT
Your appointment will only be confirmed once you’ve paid the processing fees. Make sure to pay only the
exact amount – any excess payment will be forfeited and your appointment won’t be processed if your
payment is less than the amount indicated.

Once payment has been processed, you should receive a confirmation email. If you make a mistake with
payments, don’t try to correct it with another payment! Ask for assistance from DFA through their helpdesk at
(02) 234 3488.
