Upon filling up the UPCAT Form 1, there were options as to how we are going to get our
test permits after submitting the requirements. Since our authorized school
representative will be submitting the requirements and getting the testing permits for us,
as one batch, I would like to request that my chosen option be changed wherein my test
permit be sent together with the batch of Cebu City National Science High School
through a prepaid return courier pouch provided by our guidance counselor,
