Brgy. Pagatpatan (CIGLA Village, Phase 2), Lot No. 18 Block No. 25, Butuan City,
Philippines, after having been duly sworn to in accordance with law, do hereby
depose and state:

1. That I am an awardee for lot award with the City Housing and
Development Office (CHDO) located at Brgy. Pagatpatan (CIGLA Village, Phase
2), Lot No. 18 Block No. 25, Butuan City;

2. That I am forty-one (41) years old and presently a resident of Brgy.

Pagatpatan (CIGLA Village, Phase 2), Lot No. 18 Block No. 25, Butuan City;

3. That I will partially pay at the City Treasurer’s Office in Butuan City, the
amount of Php 5,000.00;

4. That the remaining balance of Php 5,006.15 will be paid on or before the
end of July 2019;

5. That I am executing this Affidavit to attest the truthfulness of the

foregoing facts and circumstances and for whatever legal purpose and intent it
may serve.

1. That I am an awardee for lot award with the City Housing and
Development Office (CHDO) located at Brgy. Mahay (Butuan City New Villages
– Phase I Subdivision Scheme), Lot No. 05 Block No. 02, Butuan City;

2. That I am forty-six (46) years old and presently a resident of Brgy.

Mahay (Butuan City New Villages – Phase I Subdivision Scheme), Lot No. 05
Block No. 02;

3. That I will partially pay at the City Treasurer’s Office in Butuan City, the
amount of Php 5,000.00;

4. That the remaining balance of Php 17,953.00 which is payable for three
months, will be paid between the months of September, October and November
of 2019;

Subdivision Scheme (KADIYAWAN BA’AY PAGLAUK Project), Butuan City;

2. That I am fifty two (52) years old and presently a resident of Purok 5,
Brgy. Mahay, Butuan City;

3. That I will partially pay at the City Treasurer’s Office in Butuan City, the
amount of Php 8,000.00;

4. That the remaining balance of Php 14,698.12 which is payable for five
months, will be paid between the months of February to June of 2020;

5. That I am executing this Affidavit to attest the truthfulness of the

foregoing facts and circumstances and for whatever legal purpose and intent it
may serve.
