Re: Appeal for Correction of Birth Year on Driver’s

License

To Whom It May Concern:

I write to respectfully request and appeal for the correction of

the birth year appearing on my driver’s license with no. ___________.

My year of birth recorded and appearing in my aforesaid

driver’s license is 1978, which is erroneous because my correct year
of birth is 1980 as shown in my birth certificate.

In support of my appeal, I’m attaching my Affidavit of

Discrepancy, a certified true copy of my Certificate of Live Birth, as
well as my driver’s license. I am willing to pay for the necessary fines
and/or fees for the revision of records.

I therefore respectfully request that my year of birth

erroneously recorded as 1978 be corrected to 1980, and that a
replacement or renewal of my said driver’s license bearing my correct
date of birth be issued.
