Good day!

Respectfully requesting your good office for the replacement of the uniform for Tuesday,
Wednesday and Thursday into colored scrub suit similar to the colors of the uniform for
the personnel of the Local Government Unit of San Jose, Batangas. The purpose is to
make the health personnel comfortable during their work in the field and in the Main
Health Office.

Meanwhile, the use of Monday uniform of the LGU will still be followed by the health
personnel. Attached herewith, is a paper signed according to their preference ( scrub suit
or uniform).
