Request for penalty waive: Ref: Provisional Tax (05191855-01-1)

I Mr. Maandero Ndukireepo a Book keeper and founder of Integrated Investment cc,
hereby requesting for a waiving penalty of an amount of N$ 39, 800.00 on Income and
Provisional Tax returns penalty on behalf of ANN CATERING & CONSTRUCTION cc

The company has been dormant from the year of registration 2016 and the company
owner did not submit all the returns on time as he was never aware of submitting his
Income and Provisional Tax returns forms on time. The company owner promise to
submitt all the returns on time as to to date.

The company does not have a Bank Account and its currently dormant since the day of
registration.

I trust this request shall receive your favorable consideration and I am looking forward
to the waiving request as discussed above
