Dear Dr. Caballero,

It is with great concern and regret that we must inform you that on January 10th, one of our nurses
has unfortunately made an error in in her duties prior to give you service. The said nurse is now
being reprimanded and will be given just and according consequences.

There has been a series of unexpected situations occurred on that day in the Johns Hopkins Hospital
which triggered a series events that should have been handled more adequately, that caused
inconvenience towards your stay and treatment during your time here in our institute.

We deeply value giving satisfaction and assurance to our services here in Johns Hopkins Hospital.
However, due to the inconvenience, we have failed that time to give you our best service. Because
of this, we guarantee you that this will never happen again and as we have taken necessary
measures in assuring that it doesn’t in the future.

On behalf of our institute, we would like to extend and express our sincerest apologies. We take
great pride in giving our services and results, and this as well, impacts us deeply. If you have
further queries or would like to discuss toward this matter, feel free to do so as you’re more than
welcome and encouraged to.
