Sweepstakes Office through Fourth District Board Member Kerrie Keane
Shimura, Province of Cebu for the provision of PATIENT TRANSPORT VEHICLE
in the Municipality of Santa Fe.

WHEREAS, the Philippine Charity Sweepstakes Office (PCSO) is the principal

government agency for raising and providing funds for health programs,
medical assistance and services, and charities of national character;

WHEREAS, the Philippine Charity Sweepstakes Office (PCSO) has Ambulance Donation
Program (ADP) with its main objective that is to provide health care services
to indigents and other qualified patients through the provision of free
ambulance services;

WHEREAS, the Local Administration is looking after the welfare and safety of its
constituents especially the patients who need further and serious medical
attention;

WHEREAS, the availability of the Patient Transport Vehicle is of great need and help
to all patients who are referred to the mainland Cebu
WHERAS, our Internal Revenue Allotment (IRA) share and other local income are not
enough to shoulder priority projects and expenditures including the addition
of a Patient Transport Vehicle;
NOW, THEREFORE, on motion of _____________________, duly – seconded by
