AFFIDAVIT OF RESIDENCY

I, MELODINA RESTAURO VILLANUEVA, of legal age, Filipino, married, with

residence and postal address at Sitio Canapnapan I Brgy. Campo 8, Toledo City 6038
Philippines after having been duly sworn to in accordance with law, do hereby
depose and state the following that:

1. I am the petitioner in a petition for Nullity of Marriage;

2. I have resided at Brgy. Campo 8, Toledo City 6038 Philippines for more
than one(1) year already prior to the filing of the petition, in fact I was raised by my
parents at the above-mentioned address from elementary up until I finished my
secondary education;

3. As proof of my residency, I hereby attached a Barangay Certificate of

Residency, a sketched of my residence duly signed by the Punong Barangay and
Electric Bill from Cebu III Electric Coop., Inc. under the name of my late father
Gumersindo Restauro;

4. I understand that in case it is proven that I have given false information

or misrepresentation in this document or in any other documents submitted that
this may cause the dismissal of my petition and that I may be liable for perjury;

5. I am executing this Affidavit to attest to the truthfulness of the foregoing

statements.
