This is in connection with the proposed appointment of MR. RODRIGO A.

COLLANTES as SCHOOL PRINCIPAL I, which was made in violation of item 15 of CSC MC
No. 3 Series 2001 re: three (3) Salary Grade Promotion Limit Rule.

Please be informed that the said promotion was considered due to the
following justification:

I. PERFORMANCE RATING

His performance rating for the last three rating periods is 8.75-
OUTSTANDING.

II. EDUCATIONAL QUALIFICATIONS

A graduate of Bachelor in Elementary Education and Completed Academic

Requirements leading to the degree of Master of Arts in Education (MAED)
Major in Administration and Supervision.

III. RELEVANT EXPERIENCE:

He was appointed as Teacher I from October 4, 2006 to present and

designated as Officer-In-Charge of Tabion Elementary School, Del Gallego District
from September 29, 2008 – February 22, 2015.
