COIN COMPETITION AGREEMENT AND RELEASE – PARENTAL CONSENT FOR THE 2020 COMPETITION

THIS AGREEMENT AND RELEASE

is made this day of ,

BETWEEN: CHARTERED PROFESSIONAL ACCOUNTANTS OF BRITISH COLUMBIA (“CPABC”)

-and-

(name(s) of parent(s) or legal guardian(s))

I/We, (the “Guardian(s)”), am/are the legal guardian(s) of

(name of student) (the “Student”), who is under the age of nineteen

(19) and a minor under the laws of the Province of British Columbia.

For good and valuable consideration, the receipt of which is hereby acknowledged, the parties agree as
follows:

1. The Student hereby acknowledges that the Student may participate in the 2020 CPABC COIN
Competition. The Student hereby acknowledges that all information relating to CPABC’s competitions,
conferences and seminars can be found on CPABC’s website at www.gocpabc.ca.

2. The Student hereby consents to CPABC and its successors, or third parties or others working for or on
its behalf, including officers, directors, agents, and any publishers, advertising agencies, promotional
agencies or other media, storing and using the personal information of the Student submitted for the
purpose of registering with CPABC and its administering competitions, conferences and/or seminars.

3. Subject to Section 4, below, the Student hereby gives permission to CPABC and its successors, or third
parties or others working for or on its behalf, including officers, directors, agents, and any publishers,
advertising agencies, or other media to reproduce, copy, publish, or otherwise use his or her likeness
and name throughout British Columbia in video, internet, printed literature and print ads, and to
reproduce, copy, publish, or otherwise use his or her likeness and interview content, as is or as may be
reasonably retouched or edited, as taken from a photograph, videotape or any material based upon or
derived therefrom.

4. CPABC agrees that the use for which permission is given is limited to recruitment and related
purposes, and that it will make no other use of the Student’s image and related material without prior
written permission of the Student.

5. The Student agrees that CPABC has the right, in its sole discretion, to require proof of identity and/or
eligibility from the Student prior to accepting the registration. The Student acknowledges that failure to
provide such proof in a timely manner may result in the Student’s registration being denied.

6. The Student agrees and acknowledges that the Student has no claim for any compensation or benefit
from CPABC or any party acting on its behalf, arising from this agreement.

7. Provided that CPABC complies with all of its obligations as set out above, the Student agrees to
release, indemnify, and hold harmless CPABC and its successors, its members, officers, directors, and
agents, from any claims, damages, injuries, costs, or actions, including without being limited to, any
claim based on invasion of privacy, defamation, breach of intellectual property rights, or right of
publicity incurred or arising as a result of this agreement or the Student’s breach of any term of this
agreement.

IN WITNESS WHEREOF the parties have executed this document severally on the respective dates
specified below with effect from the date specified above on this Agreement and Release.
