the office of the Haran Farmers Organization, Municipality of Alfonso Lista,
Ifugao for the availment of one (1) unit multi-crop drying pavement with shed
for the association’s development.

The organization is duly registered with the Department of labor and

Employment with Registration No. CAR-2016-2-065 and was accredited by the
Department of Agriculture until March 31, 2022.

Attached herewith are the following documents for your perusal:

a. Endorsement letter of the Municipal Mayor;

b. Endorsement letter of the Municipal Agriculturist;
c. Letter intent of the President of the organization;
d. Resolution of the said organization;
e. List of members; and
f. Photocopy of the certification from the DA and registration from the
DOLE.
