3- Fixing of lifting gears spreader bar of 1200 Ton, Spreader Bar, Shackles with the lifting eye of Stripper (Lifting side) & Fixing of lifting gears of
450 Ton, slings and shackles with the lifting eye of Stripper (Tailing side)
4-Lifting of stripper with both cranes.
5-Crawling of 450 ton crane
6-Derigging
7-Positioning of load on its location
8-De rigging
9-Demobilization

Improper maintenance All the moving equipment to be greased properly.
Proper maintenance and check shall be done by E&P
department.
Daily checklist shall be maintained on daily basis and placed
with the crane.
Untrained work force All the crane crew are trained. They must bear ID,s visible .
Only certified riggers and crane operators to do the activity.
Rush in job Planned movement of all the activities to be done.
De moralization Moral boost shall be given to all the team for this activity.
Un inducted person All the persons to be trained and inducted. They are well
aware of the hazard in the work area.
Person –equipment collision Banks man shall be provided with each mobile equipment.
Un effected ERP Standby ambulance, Doctor, male nurse to be positioned in
work area.
Obstacles in the area All obstacles like planks, scaffold pipes, uneven surfaces to
be compacted and removed.
Improper planning All the work to be done as per plan
Improper positioning Proper making to e done and positioning shall be done as per
marking.
Un barricaded area Area shall be barricaded and no entry for all the unauthorised
person
Improper signage’s Signage’s indicating lifting in place shall be erected on all the
four corners of the barricaded area.
Improper traffic control Spotters shall be stationed on different location for
trespassing of the unauthorized personnel’s.
Improper access to the lifting eyes MEWP and crane (If required) shall be used for placing the
lifting gears.

Fall of person Full body harness should be used while working on MEWP.

back/Hand injury Proper posture shall be maintained /leather gloves shall be

worn at all times.
Improper alignment of rigging gears All the rigging gears shall be aligned and according to plan.

Loose shackles pins All the shackles and pins to be properly fixed

5 Lifting of Stripper with both cranes Improper crawling of 450 Ton Crawling of 450-ton crane as per plan.

Improper coordination between both supervisors and Proper coordination shall be present b/w rigging supervisors

Unawareness of the riggers about the lifting plan Riggers and rigging supervisor shall be briefed about the lift

Improper adjustment of load in its vertical position All the adjustment shall be done according to plan and with in
limits.
