their learning in a variety of ways.

Universal Design for Learning (UDL):

In this quiz, there is a setting that will read the questions aloud for students who may have a visual
impairment. Also, if a student has a hard time reading, they can have the have it read aloud so that they
understand what is being asked of them.
Describe the instructional activities that will occur PRIOR to the SRT activity and how you will introduce
the SRT activity.
Prior to the SRT Activity, students will be informed about accessing valid and reliable health information and
what the steps are in order to do that. I will inform students that they will be required to take a quiz to test
their knowledge, but they will have unlimited chances to take it.
Describe the purpose of the SRT activity (check all that apply):
☐ Assess prior knowledge ☐ Anticipatory set (Create interest in a topic) ☒ To illuminate common
misconceptions ☒ Formative assessment of content knowledge (for purpose of differentiation and
mastery for ALL students) ☐ Summative assessment of content knowledge ☐ Test preparation
☐ Survey/Poll ☒ Discussion starter ☐ “Homework” collection ☐ Other (please explain):

Briefly describe what will happen DURING the SRT activity:

During the SRT activity, the students will take the quiz as many times as they want, to test their knowledge
and see where they need improvements. The only materials needed are a way to access the internet
(desktop, tablet, computer). This activity could take however long, it should only take about 5-7 minutes to
take the quiz. Depending on how many times they take it, will be the determinant. If this is done in class along
with the online poll and discussion, it could take a whole class period or two to get it all done.

Type of questions/prompts used in this activity (check all that apply):
☒ Multiple choice ☐ Multiple select ☒ True/False ☐ Yes/No
☐ Short open-ended response or fill-in the blank ☐ Longer open-ended response ☐ Other:
If you are unable to provide a working sample of your questions, please list them below (8-10 questions
required):

Right/Wrong answers: Will there be right/wrong answers to these questions?

☒ Yes ☐ No
☐ Mixed (Some will have correct answers, other will not.)
Immediate corrective feedback: Will you pre-select correct answers to some of all of the questions and
display correct response to the class after the SRT activity?
☒ Yes
☐ No
Why or why not?
In order for students to be able to improve, it is important that they know why they got an answer wrong. So,
if students are not getting an answer, they can come in with questions about a certain topic or a particular
question that they were not understanding.
Describe what will happen AFTER the SRT activity?
After the SRT activity, there will be a google form available for students to take. It is going to consist of a quick
online poll about the quiz and see how students’ felt about it.
How will the data be used?
The data will be used as a learning point for students. In order for them to learn, they have to fail, and I think
showing the results anonymously is a great way to do that. It will be used to help me direct the discussion
about certain topics that students are struggling with. This gives students a chance to also help their peers out
by giving them their perspective on a topic that could help another student to better understand than just
having a teacher lecture about it.

Describe your personal learning goal for this activity.

This assignment is also an assessment for me as a teacher to see if students are able to retain the information
and skills that I am teaching to them. I hope to learn how effectives my teaching practices are and how they
relay to the students. I hope this helps students learn by assessing themselves and getting direct feedback on
how they did on the assignment. Then as a class, we can discuss it and see where there are some trends
within the classroom about certain topics. After taking the quiz, students are to fill out a Google Form, which
gives me direct feedback on how they thought the quiz was and any other concerns they had about it or their
understanding. Then as a class, we would discuss ways to improve and focus on encouraging students
throughout the process.
Reflective Practice:
After this lesson idea, I feel that these activities really enforce student-centered learning. This is the goal as a
teacher to have student-centered learning where students are able to assess themselves and see where they
go from there. Beyond this quiz, I would set up an online poll about the quiz and see how students felt about
it. This will then lead into a discussion regarding students’ needs of improvement and reflecting on where
they need to go next with the content.
