INSTRUCTIONS
After having your Document(s) Notarized by a Public
Notary, use this form to legalize/fictionalize the document with the
State, e.g. Secretary of State. The State may provide their own
form(s) for submission and request. For instance, on a national
level, United States, Inc - Office of Authentication uses Form
DS-4194.
As this is a NOTICE rather than a Request, the document(s)
is/are technically legalized with and without any processing by the
State.

This process should legitimately legalize any signed and

Typically, States will only issue Apostilles and Certificates of
Authentication for documents destined to be used OUTSIDE the
Nation.
Apostille or Certificate of Authority?
The Hague Convention of 1961 Abolishing the Requirement
for Legalization of Foreign Public Documents is a convention (set of
model statutes) that signatories, according to the convention,
recognize each others legal documents as legalized/fictionalized
without the need to legalize the documents with the
Embassy/Consular of another Foreign Nation.
By being party to this Convention, nations may exchange
Negotiable Instruments (as defined by U.C.C. - Article 3) and other
documents without the need to legalize the documents. This
includes Birth Certificates, Death Certificates, Marriage Certificates,
and Vehicle Titles, among other things.
• Documents destined for use in a Foreign Country that is
party to this Hague Convention should be issued an Apostille.
• Documents destined for use in a Foreign Country that is
NOT party to this Hague Convention should be issued a Certificate of
Authentication.

the document to the “Holy See” [Sea], the document is legalized for

all entities upon the Ships of State practicing Law of the Sea,
otherwise known as Statutory Law and Commerce.
Through the Papul Bulls Romanus Pontifex, Aeterni Regis,
and Convocation the Vatican/Holy See* is the top-most
Corporate/Legal entity that controls all legalized assets [cestue que
vie trusts].
(*The Vatican was established as a separate “State” by
Benito Mussolini, the Fascist, for the Vatican’s support of Hitler and
the Nazis.)

All major Nations, as of this writing, are members of this
convention. Many of the small nations are not party to this
convention.
This convention prevents Lawful documents from becoming
legal documents before being process. This allows legal entities to
ignore lawful documents as they have not been
legalized/fictionalized.

HOW TO FILL OUT THIS FORM
Fill in the date of the letter.
Fill in the Country of Destination based upon how you
intend to use the legalized/fictionalized document.
Fill in what kind of document you wish to receive, Apostille
or a Certificate of Authentication based upon its destination location
of use.
Enter the number of documents to authenticate, the cost
per document, and total cost multiplying these numbers together.
The payment may be waived by U.S. Public Law 73-10 (a)
- Prohibition of payment of Debt, though this may not recognized by
the State/Nations. This is applicable upon and within the United
States, Inc. Nearly every national government is a corporate
franchise of and by the United States, Inc. While technically HJR
192 applies in all fiat money nations, it is not guaranteed to be
recognized, even within the United States, Inc.
Indicate how you intend to receive the documents back
from the Authentication Authority. Please include the self addressed
return envelope with postage or a mail carrier label in the package
to the Filer.
Enter your information as necessary. While not known to
be required for filing, some states may require some of this
information.
Where legally necessary, fill in the Filers information of
their name and where the document is to be authenticated.
Sign and Date. To sign digitally, prepend the text “/s/”
before the signature. (e.g. “/s/ John Q Doe©”, “/s/ John-Q: Doe©
representing DOE, JOHN Q.©”)
HOW TO FILE
Once the data is properly entered onto the form, the
documents can be sent for Document Authentication. The package
should include the following:

• The sealed document to be authenticated. The document
to be authenticated needs to be signed and sealed with a Public

Notary, Sheriff, Clerk, or other legal authority prior to being sent to
the State/Nation. Do not sign your document until present with a
Public Official who can stamp your document with their Seal.
• A Self Addressed return Envelop with postage or a
pre-paid Mail Carrier Label.
• Payment, unless claiming a waiver under H.J.R. 192.
It is recommended to use an ANSI A/A4 Envelop (9”x11”,
216mm x 279mm) for both sending and receiving.
