Log of Hours: (Completed)

In a table format, the final log of hours and activities should include the date, time, hours worked,
and detailed description of activity, task, responsibility, etc. for each day worked (see example). It is
your responsibility to provide your preceptor with your log of hours and they must sign off on your
total number of hours worked.
