CONSENT FORM

This questionnaire is with relation to a research being conducted by students of Christ (Deemed
to be University), BGR Campus for an assignment in the realm of Industrial Psychology. The
information collected is strictly for academic purposes and the same is confidential. This would
not be lent out to any other organisation. The choice of response is completely dependent on the
respondent. This study is validated by the University.

participate in the research being conducted by the students of Christ (Deemed to be University)
and am aware about the purpose of the document. The following parameters have been specified
to me prior to the research.

1. I have been given sufficient information about the project. The purpose of my
participation as an interview in this project has been explained to me.

3. I am aware of the fact that the researcher is making notes with respect to the interview.
