Important steps you need to take before your request can be completed:

1. Gather the following information:

a. Type of natural disaster
b. Did the disaster affect your home or place of employment?
c. Date of disaster
d. Date you were affected by the disaster
2. Go to the Contact Us option and use one of the following methods to apply:
a. Your best option - Send us an email with the above information
b. Call us toll free and provide us with the above information
c. Provide the above information along with your name and account number to the address or
fax number below

If a co-maker exists on any loans:

 You and your co-maker must be eligible for forbearance during the same time period
 Have your co-maker apply for the same or different forbearance type for the same time period using
a separate form
