QUALIFIED THEFT

DAENERYS STORMBORN TARGARYEN, of legal age, single and a

residenct of Brgy. Dragonstone, Laoag City, Philippines personally

appeared at this station and requested to put on record that on or about 8:00 P.M.

of January 30, 2018, she was driving northbound along Ablan Avenue,

maintaining a moderate speed on her proper lane on well paved and

asphalted pavement. Upon reaching the vicinity of University Belt

(fronting 7/11 store) and while in the middle of crossing the

intersection, a white Toyota Fortuner eastbound suddenly and without

warning bumped the left side causing damage to its rear bumper; Due

to the collision, the Toyota Vios driven by her violently swerved

towards a concrete barrier. Immediately, the she was rushed to Ilocos

Norte Provincial Hospital for immediate medical attention. Plaintiff

sustained multiple physical injuries and her left leg was badly injured

and had to be amputated as a result of the collision
