Please send 3 PP (Personal Particular) forms for re-verification of your Police Verification.

Please submit self attested photocopies of the relevant documents .. The originals of such
documents need to be carried while visiting Passport Office between 10:00 AM to 1:00 PM from
Monday to Friday.
Please make sure to mention your file number and date on top of each page/document . If the
service is rendered, please ignore this letter or please treat this letter as cancelled.
