PERMISSION TO CONDUCT EYE SCREENING EXERCISE FOR THE STAFF OF THE
AMPONSAH-EFFAH PHARMACEUTICALS
We humbly wish you could use your office to grant us the permission to conduct eye
examination program which encapsulates glaucoma test, cataract assessment, screening for
refractive errors etc. for the workers. We believe that diagnosing and treating significant eye
conditions we find as well as making referrals to tertiary eye centers will improve work
efficiency for the workers.
We promise to provide a written report with statistics on the outcome of the screening with
respect to the eye conditions we identified if our request is granted. We hope to hear from you
