Coast Academy Parent Involvement Evaluation

Please use this self-survey to evaluate the parent involvement program within Coast Academy.
Please print or fill out the form electronically to evaluate either your student’s classroom or the
overall academy parental involvement program. Please use the following Likert Scale to evaluate
the program.

Welcoming Environment
