    AFFIDAVIT

    I,BALLA SEETHA W/o Late APPARAO, Hindu, Aged 50 Years, Resident


    of Srinagar Colony do here by solemnly affiram and state on oath as
    follows

    I Submit that my husband BALLA APPARAO died on 06-07-2011 at


    my residence, due to illiteracy and lack of knowledge, I did not
    inform the death of my husband to the concerned authorities for
    recording the same.

    Therefore I request your kind authority to note the death of my


    husband BALLA APPARAO as on 06-07-2011 and to issue death
    certificate to me.

    The contents stated above are true and correct to the best of my
    knowledge, belief and information.
