Instructions: Each year MDT students must meet with both the Academic Advisor and their Faculty
Advisor to review their graduation plan. If you do not meet with both the Academic Advisor and Faculty
Advisor before March 1, a registration hold will be placed on your student account and will remain in
effect until you have completed both meetings. To avoid having a registration hold placed on your
account, please take the following steps:

1. Print this form and fill in the grad plan as best you can IN PENCIL. You may utilize the BYU
Catalog and classschedule.byu.edu to find the time offered (if future semesters are unavailable,
use previous semesters).
2. Schedule an appointment with your academic advisor, Randa Alvord by calling 801-422-3777 or
by coming to D-444 HFAC. In your appointment, you will review your graduation plan and make
any necessary changes, as well as list any items that need to be reviewed with your Faculty
Advisor. (Please bring any substitution/transfer equivalency approvals.)
3. After meeting with the Academic Advisor, schedule an appointment with your Faculty Advisor to
review your graduation plan. You will need to bring your Progress Report (academic record) to
this appointment.
4. After the Faculty Advisor signs, return this form to D-444 HFAC or email as a PDF attachment to
Randa_Alvord@byu.edu so it can be placed on your file.
