Re: Discharge of ROBERTO ROCHO DELA PEŇA

Dear Dr. Del Mundo:

This refers to the case of ROBERTO ROCHO DELA PEŇA who was admitted in this Center
for treatment and rehabilitation on July 2, 2017 thru VTP.

The subject has been making positive progress toward most of the goals on his treatment plan.
After he underwent through the prescribed treatment program, behavioral changes were
apparent. He responded well to redirection and reported that he had successfully avoided
altercations between his co-patients. He has attended activities regularly and conformed to the
regulations of the program. He has recognized internal and external motivation and has devised
strategies to deliver assigned tasks successfully. He developed coping mechanisms that were
appropriate to the problems he faced in the Therapeutic Community and within his family
relations. Furthermore, he appears to be in overall good physical health. He was given a pre-
discharge planning session with his supportive family. Consequently, he was able to identify
possible triggers for relapse and was able to make plans for his reintegration to the society.

WHEREFORE, in view of the aforementioned, subject ROBERTO ROCHO DELA PEŇA is

respectfully recommended for DISCHARGE from this Center.
