Nurse : “Good morning Miss”.”

Patient : “Good morning nurse”.”

Nurse : “My name nurse febi work in this room from 7 am to 2 pm. well I need to collect your
personal data. I’m going to ask you some question”.”

Petient : “Of course, nurse”.”

Nurse : “Ok Miss, what is your complete name”?”

Patient : “My name is Yayang hartini”.”

Nurse : “How old are you” ?”

Patient : “I’m 19 years old”.”

Nurse : “Where do you live”?”

Patient : “I live in Sukabumi city”.”

Nurse : “What’s your number phone”?”

Nurse : “What’s your religion”?”

Patient : “My religion is Islam””

Nurse : “Are you married”?”

Patient : “No, I’m single, nurse”.”

Patient : “Yes, I have health insurance BPJS” .”

Nurse : “What is your occupation”?”

Patient : “I’m a collage student””

Nurse : “What is your next of kin”?

Patient : “My parents”

Nurse : “What makes you come to this hospital”?

Patient : “I come this hospital because I feel pain when swallowing since 3 days ago”

Nurse : “What's your problem”?”

Patient : “My throat is very painful and I feel loss of appetite””

Nurse : “Now I want to asses your mouth please do my intruction, Are you ready” ?

Patient : “oke, I ready””

Nurse : “Please smile””

Patient : “(doing the instruction)”

Nurse : “Would you show your teeth”?

Patient : “(doing the instruction)”

Nurse : “I want to check your throat now. Can you open your mouth”?

Patient : “yes...AAAAAAA””

Nurse : “Oke, I have finish to asses your personal data and assesment your mouth, you may
have Faringitis”

Patient : “How is it nurse”?

Nurse : “Don't be afraid. You will be fine, now doctor will to write prescription for you”.

Patient : “Thank’s you very much Nurse”.”

Nurse : “you’re welcome , i am permission, later If you need something you can call me in
nurse room”.”

Patient : “Oke nurse”.”
