AFFIDAVIT OF LOSS

I, RYAN I. IDANAN, Filipino, of legal age, married and a resident of Cavinitan, Virac,
Catanduanes, after having been sworn to in accordance with law, hereby depose and state that:

1. I am the subject teacher in Edukasyon sa Pagpapakatao for Grade 7, Grade 8, and Grade 10
students and adviser of Grade 9 Integrity students at Catanduanes National High School
located at Constantino, Virac, Catanduanes;
2. At the start of the school year, I am the one in-charge of distributing the required handbooks
for Edukasyon sa Pagpapakatao 8, Edukasyon sa Pagpapakatao 10, and Science Learners
Module 9 for my students in four sections as follows:

3. However, the aforementioned 34 handbooks were lost while under the custody of my Grade
8, Grade 9 and Grade 10 students when typhoon Nina hit the province last December 25,
2016;
4. Said handbooks could not found notwithstanding diligent efforts to locate the same;
5. I am executing this affidavit to attest to the truth of the foregoing statements and for whatever
legal purposes this may serve.
