Re: Complaint-Affidavit against Mr. John Cordova, Revenue Collector I, under the
Office of the City-Treasurer

Dear Sir/Madam:

I respectfully seek the assistance of your Honorable Office in the filing of the instant
administrative complaint against Mr. JOHN CORDOVA, Revenue Collector I of the Office of the City
Treasurer of Kabankalan City, Negros Occidental as your Honorable Office may deem proper on the
basis of the attached Complaint and annexes correspondingly marked, to wit:

 Annex A – Letter-Complaint dated March 23, 2020;

 Annex B – Certified True Copy of the Incident Record Form issued by the Philippine National
Police;

I trust that you will find all documents attached hereto in proper order, and I respectfully
request that after the conduct of the preliminary investigation, the herein respondent JOHN CORDOVA,
be found liable for Grave Misconduct or as may be found by your Honorable Office, so that the
corresponding penalties may be meted.

any clarificatory hearing that may be found necessary in pursuing justice against the respondent.
