Notice of Incomplete

This office has reviewed your proposal for sewage disposal and water supply systems for the
above referenced project. The following will be required prior to any further processing of the
application for approval to construct:

(1) Submit NYSDEC Wild, Scenic and Recreation Rivers Permit.


(2) Submit a Water Availability Letter from Riverhead Water District & a Sewer
Availability Letter for the Calverton Sewer District.
(3) Lot 1 appears to be located within the Central Pine Barrens Core. Provide
approvals from the Pine Barren’s Commission for review.
(4) County SEQRA Determination pending.
(5) Submit a Certificate of Authorization to perform engineering services from the
design professional.
(6) Covenants to be determined if necessary upon resubmission for water main
extension, undeveloped open space (lot 1), and water/sewer easements.
(7) Tax lot 7.29 (~4 acre parcel owned by Riverhead Water District) appears to have
been split from tax lot 7.33 without SCDHS approval and will need to be included as
part of this subdivision application. Revise the overall metes & bounds to include lot
7.29 and update the site data accordingly (i.e lot area, key map, SCDHS stamp, etc.).
In addition, the NW corner of the lot 7.33 appears to have an 85’x148’ section of land
that may have been dedicated to NYSDOT; clarify and provide any additional
documents to detail the land transfer.
(8) Show all existing buildings, water, and sanitary systems on the subject property.
(9) Indicate on map if the sewer easements shown to tax lot 4 & the NYS property
(lot 7.30) are existing or proposed. If existing, provide a copy of the recorded
easements for review.
(10) Show conceptual water & sewer connections on map for all lots that may be
developed in the future. Indicate the distance the water/sewer mains are to be
extended if necessary. If easements are required, then show the proposed locations
on map with metes & bounds descriptions.
(11) Submit 4 revised maps with original PE/LS signatures/seals & the design
professional statement signed/dated.
