I, ABEGAIL BACOLOD BERNAL, of legal age, Filipino, with residence

address at 218 Via Ferrea St., Caridad, Cavite, Philippines, after having
been sworn to in accordance with law depose and state:

1. That, I underwent normal vaginal delivery under Dra. Shaula

Ram-Pulido on April 28, 2015 at Cavite Medical Center, as
evidenced by a Certification dated June 28, 2019 issued by the
said medical center, hereto attached for reference;

2. That I am executing this affidavit to attest to the truth of the

foregoing and for whatever lawful purpose that it may serve.
